Determinants of Dropout From Correctional Offender Treatment

Research indicates that approximately one third of offenders admitted to social-therapeutic correctional facilities in Germany fail to complete treatment and that treatment dropout is linked to higher recidivism in both sexual and violent offenders. The purpose of this study was to examine determinants of treatment dropout in a social-therapeutic correctional facility in Germany. The sample consisted of 205 incarcerated adult male offenders (49.8% sexual, 38.1% non-sexual violent) admitted to correctional treatment. Completers and dropouts were compared on variables pertaining to demographics, offense type, substance abuse, psychopathy, risk, and protective factors. Univariate analyses showed that treatment dropouts demonstrated significantly higher scores on measures of risk and psychopathy and lower scores on protective factors. Logistic regression analyses identified unemployment, non-sexual violent index offense, higher risk scores (HCR-20), and Facet 1 (interpersonal deficits) of the Psychopathy Checklist-Revised (PCL-R) as significant predictors of treatment dropout. Surprisingly, substance abuse disorder was a negative predictor of dropout. With the exception of substance abuse, the results support the notion that treatment dropouts represent a group of high-risk offenders with particular treatment needs. Practical implications and suggestions for further research are discussed.

Resumo.A definição e a identificação das principais características e traços de indivíduos psicopatas têm sido, há várias décadas, objeto de diferentes áreas de estudo. No âmbito forense, a psicopatia é um termo bastante discutido, sendo fortemente relacionado a criminalidade. No contexto de tratamento, observa-se na literatura o emprego crescente de estratégias cognitivo-comportamentais, as quais têm apresentado diversos resultados. Avaliar as principais estratégias cognitivas e comportamentais empregadas no tratamento da psicopatia a partir dos resultados de estudos empíricos, considerando as evidências de eficácia. Foram consultadas as bases de dados Scielo, PubMed, Lilacs e o portal Periódico CAPES. Critérios de inclusão: 1) artigo empírico original; 2) amostra de indivíduos diagnosticados com psicopatia ou que apresentem traços ou características psicopáticas; 3) utilizar estratégias cognitivas e comportamentais; 4) publicado entre 2014 e 2019. Foram identificados 848 artigos que após inclusão dos filtros, leitura dos títulos e resumos, 23 trabalhos foram rastreados. Posteriormente, considerando os critérios de inclusão, sete artigos foram incluídos na análise. As intervenções comportamentais e cognitivas apresentaram resultados diversos. Porém, independentemente dos resultados, os artigos analisados apresentaram consideráveis limitações, isso interferiu de forma negativa a evidência de eficácia. Os psicopatas podem responder e se beneficiar dos tratamentos comportamentais e cognitivos. Porém, o campo ainda apresenta poucas pesquisas randomizadas, exibindo as mesmas limitações apontadas em trabalhos de revisões

A dilemma for corrections practitioners is treatment for pretrial detainees. They are innocent until proven guilty and are not required to take treatment, but many may benefit from intervention. To assess the general level of treatment interest and potential differences, a sample of 221 male remand and sentenced Canadian provincial prisoners completed several Client Evaluation of Self and Treatment (CEST) scales. Prisoner treatment motivation and its correlates were assessed by examining univariate, bivariate, and multivariate effects for demographic attributes, legal factors, risk, perceptions of personal/family/pressure for treatment, and depression. It was found that about 36% to 40% of study subjects expressed moderate to strong motivation for treatment. Age, pressure, and depression were the only correlates consistently associated with treatment motivation. There were no differences found between remand and sentenced prisoners. Results indicated that pretrial detainees have a definite interest in undertaking programming.
