The impact of palliative irradiation on the management of patients with acquired immune deficiency syndrome.

Abstract: We have studied the natural history of the acquired immune deficiency syndrome (AIDS), and the part that irradiation plays in the management of this devastating and fatal disease. The radioresponsiveness of the two most common malignancies associated with AIDS, Kaposi's sarcoma (KS) and malignant lymphoma (ML), has been demonstrated. We have documented satisfactory time-dose relationships for the management of multiple manifestations of KS, including cutaneous and deep subcutaneous extremity involvement, visce… Show more

“…As a result of the ensuing obvious cosmetic deformity, treatment is frequently sought, despite reassurances of the cysts' benign nature. Traditionally, available therapy has been limited to either repeated percutaneous aspirations, 4–6 surgical excision including enucleation or superficial parotidectomy, 1,4,7,8 radiation therapy, 6,9 and rarely, medical therapy with zidovudine (AZT) 6,7 . However, none of these treatment modalities have been able to consistently control the cysts with reasonable degrees of morbidity.…”

“…AIDS-KS is known to be responsive to HAART, and control of HIV load is the primary goal in the treatment of AIDS-KS [46]. Pain, edema and cosmetic concerns arising even from slowly progressive disease can be addressed with local therapies, such as irradiation [47,48] or excision, alitretinoin gel (a retinoid approved for topical application) [49], IFN-α [50], or experimental therapies [51], such as novel angiogenesis inhibitors (reviewed in [46,[52][53][54]). Advanced or quickly progressing disease can be treated with systemic cytotoxic chemotherapy.…”
