The impact of healthcare associated infections on mortality and length of stay in Singapore—A time-varying analysis

Abstract: Objective: Methods that include the time-varying nature of healthcare-associated infections (HAIs) avoid biases when estimating increased risk of death and excess length of stay. We determined the excess mortality risk and length of stay associated with HAIs among inpatients in Singapore using a multistate model that accommodates the timing of key events. Design: Analysis of existing prospective cohort study data. Setting: Seven public acute-care hospitals in Singapore.… Show more

“…Actual costs were not within the scope of this study. However, shortening the ICU length of stay can benefit patients and reduce costs, as longer hospital stays are associated with nosocomial infection, death, delirium, and long-term cognitive impairment [ 42 , 43 ].…”

“…4,5 Estimates of the prolongation of stay due to HAIs were obtained from a prospective cohort study, which adjusted for the time-dependent and competing risk nature of HAIs. 6 Bed cost was estimated using the 'cost accountant' approach, which considers historical expenditures against some measure of activity. Uncertainties around the input variables were modeled using Monte Carlo simulations consisting of 1,000 iterations (TreeAge Pro 2020, TreeAge Software, Willianstown, MA).…”

“…Klebsiella pneumoniae is a Gram-negative opportunistic pathogenic bacterium and common cause of hospital- and community-acquired urinary tract infections, sepsis, and pneumonia [ 1 ]. In recent decades, cases of community-acquired suppurative liver abscess, caused by K. pneumoniae and complicated with metastatic meningitis and endophthalmitis have emerged worldwide, particularly in Asia [ 2 4 ], and the clinical picture is rapidly progressing.…”
