The impact of esophagogastric varices on the prognosis of patients with hepatocellular carcinoma

Abstract: Whether or not esophagogastric varices (EGV) could determine the outcomes of patients with hepatocellular carcinoma (HCC) is still unclear. A total of 990 treatment-naive HCC patients who received an esophagogastroduodenoscopy at the time of HCC diagnosis were retrospectively enrolled. The factors in terms of prognosis were analyzed by Cox proportional hazards model and propensity score matching analysis. Among the enrolled patients, 480 (48.5%) patients had EGV. Patients with EGV had a significantly lower cum… Show more

“…*The were 23 (60.5%) and 92 (73.6%) patients with high risk EVs who had EV prophylaxis in SR and RFA groups, respectively (p = 0.179). The presence of EGV, is a surrogate for CSPH and has been validated as an independent factor for poor prognosis among patients with HCC 18,31,32 . Several studies show that the incidence of developing liver decompensation after SR is high among HCC patients with CSPH, which would increase the risk of mortality 24,25,33 (Table 5).…”

“…The majority of patients with HCC have underlying advanced chronic liver disease or even portal hypertension [19], which could make the adoption of treatment strategies and the prediction of outcomes more complex. In addition to the tumor factors, the grade of liver function is also crucial in determining the outcomes of HCC patients, especially of those with early-stage tumor [20].…”

“…In most countries, hepatocellular carcinoma (HCC) represents the traditional surgery to more effective therapies, such as liver transplantation, chemotherapy, radiotherapy, and thermotherapy, the survival rates for HCC are unfortunately poor owing to the high invasive ability of HCC cells [2]. Therefore, it is urgent to explore the genetic mechanisms of HCC and to search for personalized treatment options.…”
