The impact of COVID-19 and social avoidance in urgent and emergency surgeries – will a delay in diagnosis result in perioperative complications?

Abstract: OBJECTIVE:The sudden COVID-19 outbreak has changed our health system. Physicians had to face the challenge of treating a large number of critically ill patients with a new disease and also maintain the essential healthcare services functioning properly. To prevent disease dissemination, authorities instructed people to stay at home and seek medical care only if they experienced respiratory distress. However, there are concerns those patients did not seek necessary health care because of these orientations. “…In the United States, healthcare systems prioritized care and modified protocols to mitigate COVID-19 transmission and resource burden. 1 , 2 Hospital systems reallocated health resources to accommodate COVID-19 patient care, resulting in the postponement and/or cancellation of operations. Some institutions, like ours, implemented a tiering system to triage surgical priority based on recommendations from the American College of Surgeons.…”

“…This occurred because there was a need to release more beds and greater availability of mechanical ventilator in case of increased demand for them. In addition, the pause in elective surgery aimed at preventing contamination of patients undergoing procedures with low morbidity and mortality 7 . Thus, hernia repairs, which are only performed in the context of an emergency when it is not possible to manually reduce the hernia content 4 , were rescheduled for a more opportune moment in the pandemic.…”
