The Impact of Bedside Behavior on Catheter-Related Bacteremia in the Intensive Care Unit

Abstract: The success of an educational program in July 1999 that lowered the catheter-related bloodstream infection (CRBSI) rate in our intensive care unit (ICU) 3-fold is correlated with compliance with "bestpractice" behaviors.Design: Before-after trial.Setting: Surgical ICU in a referral hospital.Patients: A random sample underwent bedside audits of central venous catheter care (n = 187). All ICU admissions during a 39-month period (N = 4489) were prospectively followed for bacteremia.Interventions: On the basis of … Show more

“…17 Adherence to these guidelines has been included in successful quality improvement initiatives. 4,5,18 Compared to internists' adherence to recommendations for infection control reported in another survey, 10 residents who attended our educational session reported more use of large sterile drapes (94% vs. 35%) or masks (75% vs. 66%); however, they were less likely to use a sterile gown (50% vs. 72%). Use of a large sterile drape is common in our hospital, likely because the drape is included in the central venous catheter package.…”

“…Infection rates decreased from 4.9 to 2.1 cases per 1,000 catheter days. The effectiveness of a self-study module combined with posters, fact sheets and lectures has since been further demonstrated in the ICU setting, with several studies showing significant decreases in catheter-related bloodstream infection (CRBSI) [17,[26][27][28], VAP [18,29] and hand hygiene adherence [30].…”
