The impact of antibiotic resistance on the efficacy of three 7‐day regimens against Helicobacter pylori

Abstract: One-week RCM is the best regimen to eradicate H. pylori in our geographical area. This seems to be linked to the better ability of RCM compared to OCM and BAM in overcoming the high prevalence of H. pylori resistance to both metronidazole and clarithromycin in our region.

“…Earlier studies conducted in Italy showed that primary resistance to clarithromycin was high (18.4% -26.7%) in Central and South Italy and similar to 23% reported in another Italian study [9][10][11][12]. These percentages are higher than 1.8% and 14% found in studies conducted about ten years ago [13,14] suggesting an increase of resistance rate over time. In addition, these resistance rates may be higher due to the reported discordance between the two methods used to establish the resistance: E-Test and Real Time PCR, the latter detecting more resistant strains [7].…”

“…Reports have shown that about 3-14% [12][13][14] of H pylori isolates are resistant to clarithromycin, 12-44% [13][14][15][16] are resistant to metronidazole. But no H pylori was found primary resistant to amoxicillin.…”

“…Moreover, one sample from the antrum and one from the corpus were taken for culture, which was performed using the previously described methodology. 11 In agreement with the guidelines for clinical trials in H. pylori infection, 6 patients were considered to be infected if they were positive on both rapid urease testing and histological examination, or if only culture was positive for H. pylori, in relation to its absolute specificity. All H. pylori-positive patients underwent 1-week eradication treatment combining proton pump inhibitors or ranitidine bismuth citrate with two of the following three antibiotics (metronidazole, clarithromycin and amoxicillin).…”
