Several putative biomarkers have been suggested for identifying murine follicular stem cells; however, human hair follicles have a different pattern of biomarker expression, and follicular stem cell isolation methods have not been established. To isolate a stem cell population applicable to clinical settings, we conducted a comprehensive survey of the expression of stem-cell-associated (K15, CD200, CD34, and CD271) and other biomarkers (K1, K14, CD29, and CD49f) in immunohistological sections of the human epidermis and follicular outer root sheath (ORS). We also examined freshly isolated and cultured epidermal or follicular cells with single-and multicolor flow cytometry or immunocytochemistry. After sorting cells by CD200, CD34, and forward scatter (FSC) values (cell size), colony-forming assays were performed. We found that biomarkers were differentially expressed in the epidermis and ORS. Basal bulge cells were mainly K15 þ CD200 þ CD34 À CD271 À , and suprabasal cells were K15 À CD200 þ CD34 À CD271 À . We categorized follicular cells into nine subpopulations according to biomarker expression profiles. The CD200 þ CD34 À bulge cells had much higher colony-forming abilities than the CD34 þ population, and were divided into two subpopulations: a CD200 þ CD34 À FSC high (K15-rich, basal) and a CD200 þ CD34 -FSC low (K15-poor, suprabasal) population. The former formed fewer but larger-sized colonies than the latter. Follicular epithelial cell cultivation resulted in loss of K15, CD200, CD34, and CD271 expression, but maintenance of K14, CD29, and CD49f expression. We found that the bulge contained two populations with different localizations, cell sizes, and colony-forming abilities. We showed that K15, CD200, CD34, and CD271 were useful biomarkers for characterizing freshly isolated human follicular epithelial cells in diverse stages of differentiation.

Objective: The etiopathogenesis of lichen planopilaris is not known well and and it is thought that inflammatory infiltrate heads towards to the region adjacent to stem cells on bulge region of pilosebaceous unit. In this study, it was aimed to describe CK15, CD34, nestin and CD200 expression patterns in patients with lichen planopilaris in order to contribute describing which parts or cell types in the follicle were damaged. Methods: In this study, affected and healthy scalp specimens of 10 lichen planopilaris patients and scalp specimens of 9 patients undergoing surgical excisions due to sebaceous cysts were included. Routine immunohistochemical analysis of CK15, CD34, nestin and CD200 primer antibodies were performed to all tissue samples. Stained slides were graded according to expression intensity from no expression (-) to strong expression (++). Results: CK15, CD34 and CD200 expression patterns were similar in controls and patient lesional samples. Nestin expression only decreased in the sub-bulge area in patient lesional samples. Conclusion:The absence of apparent differences in the CK15, CD34, nestin and CD200 expressions of affected scalp samples of patients suggests that these markers may display different expressions at different stages of the hair follicle cycle and cell differentiation and further studies are necessary to figure out whether these expressions are changed with the activity of the disease. Bu çalışmada liken planopilarisde folikülde hangi bölgelerin, ya da hangi yapıdaki hücrelerin hasara uğradığını açıklamaya katkıda bulunmak amacıyla CK15, CD34, nestin ve CD200 immünohistokimyasal ekspresyon paternlerini tanımlamak hedeflenmiştir. Yöntemler: Çalışmaya klinik ve histopatolojik olarak liken planopilaris tanısı almış 10 hastanın sağlam ve hastalıklı saçlı deri örnekleri ve saç hastalığı bulunmayan, sebase kist nedeniyle cerrahi operasyon geçirmiş 9 kişiden elde edilen saçlı deri doku örnekleri alındı. Tüm örneklere CK15, CD34, nestin ve CD200 primer antikorları ile rutin immünohistokimyasal işlem uygulandı. Boyanmış kesitler ekspresyon şiddeti; ekspresyon yok (-) ile kuvvetli ekspresyon (++) arasında derecelendirildi. Bulgular: CK15, CD34 ve CD200 ekspresyon paterni kontroller ve hasta örneklerinde benzerdi. Nestin ekspresyonu hasta örneklerinde sadece tümsek altı bölgede daha zayıftı. Sonuç: CK15, CD34, nestin ve CD200 ekspresyonlarında hasta örneklerinde belirgin farklılıklar göstermemesi, bu belirteçlerin kıl folikülü siklusunun farklı evrelerinde ve hücre differansiyasyonlarına göre farklı ekspresyonlar gösterebileceğini ve hastalığın aktivitesi ile ilişkili ekspresyonların değişip değişmediğinin daha geniş serilerde değerlendirilmesi gerektirdiğini düşündürmüştür.

Epithelial hair follicle stem cells (eHFSCs) are required to generate, maintain and renew the continuously cycling hair follicle (HF), supply cells that produce the keratinized hair shaft and aid in the reepithelialization of injured skin. Therefore, their study is biologically and clinically important, from alopecia to carcinogenesis and regenerative medicine. However, human eHFSCs remain ill defined compared to their murine counterparts, and it is unclear which murine eHFSC markers really apply to the human HF. We address this by reviewing current concepts on human eHFSC biology, their immediate progeny and their molecular markers, focusing on Keratin 15 and 19, CD200, CD34, PHLDA1, and EpCAM/Ber-EP4. After delineating how human eHFSCs may be selectively targeted experimentally, we close by defining as yet unmet key challenges in human eHFSC research. The ultimate goal is to transfer emerging concepts from murine epithelial stem cell biology to human HF physiology and pathology.
