Airway clearance therapy is an important treatment strategy in patients with respiratory disease and impaired mucociliary transport (e.g. cystic fibrosis [CF]) [1-4]. Airway clearance techniques (ACTs) supplement the mucociliary clearance system in the presence of disease-related impairment [2], and are important for maintaining respiratory health [4]. Current airway clearance interventions are based mainly on physical or mechanical measures to move air behind mucosal obstruction, and modulation of expiratory airflow to enhance movement of secretions to more proximal airways for more effective clearance [2]. ACTs in obstructive lung diseases encompasses very various interventions including 1/ manual chest physiotherapy (CP): conventional CP (postural drainage, percussion, vibration, forced expiration technique, directed cough), autogenic drainage and active cycle breathing technique, and 2/ airway clearance devices (ACD) as Positive Expiratory Pressure (PEP), oscillating PEP and high frequency chest wall oscillations [2, 5]. ACTs have been shown to have beneficial short-term effects in increasing transport mucus away [5, 11] with no clear differences in effectiveness between techniques [6-10,12,22].Whatever the approach, the overall goal of airway clearance therapy is to improve mucus clearance, thus decreasing the risk of bronchial infection, attenuating disease progression and lung function decline, and improving quality of life [4,[11][12][13]. However, strategies such as CP are intensive and time-consuming, may require assistance from a physiotherapist or caregiver, and can be uncomfortable for patients [5]. Furthermore, these strategies require time and effort from the patient and their caregivers, and adherence to CP may be suboptimal in adults with CF [14], and compliance with CP is comparatively low in this group (53%) [15].

Modelo do estudo: Revisão sistemática. Objetivo: Avaliar se os recursos de fisioterapia respiratória modificam a impedância pulmonar avaliada pelas técnicas de oscilações forçadas (TOF’s) em adultos com distúrbios ventilatórios obstrutivos. Método: Trata-se de uma revisão sistemática conduzida nas bases de dados PubMed, Scielo, Lilacs e PEDro, utilizando-se a seguinte estratégia de busca: Obstructive pulmonary disease OR chronic obstructive pulmonary disease OR COPD OR Bronchiectasis OR Bronchial diseases OR Cystic fibrosis OR mucoviscidosis AND impulse oscillometry OR forced oscillation techniques AND physical therapy OR physiotherapy AND Airway Resistance. Resultados: De um total de 216 artigos, apenas 3 foram incluídos. As amostras totalizaram 64 participantes, incluindo adultos com fibrose cística (FC), doença pulmonar obstrutiva crônica (DPOC) e bronquiectasias. Dois estudos utilizaram o flutter® e um realizou drenagem autógena. A impedância pulmonar foi avaliada em duas pesquisas pelo sistema de oscilometria de impulso (IOS) e em um pela TOF modificada (TOF-m). Houve redução da resistência em várias frequências, sendo que as mais frequentes foram a resistência total (3/3), a resistência central (2/3) e a periférica (2/3). Além disso, a reatância pulmonar apresentou resultados conflitantes. Conclusão: Evidenciou-se que os recursos fisioterapêuticos utilizados para remoção de secreções das vias aéreas modificam a impedância pulmonar avaliada pelas TOF’s em adultos com DVO, sendo a resistência total das vias aéreas o parâmetro com maior ganho terapêutico.

BackgroundAutogenic drainage is an airway clearance technique that was developed by Jean Chevaillier in 1967. The technique is characterised by breathing control using expiratory airflow to mobilise secretions from smaller to larger airways. Secretions are cleared independently by adjusting the depth and speed of respiration in a sequence of controlled breathing techniques during exhalation. The technique requires training, concentration and effort from the individual. It is important to systematically review the evidence demonstrating that autogenic drainage is an effective intervention for people with cystic fibrosis. ObjectivesTo compare the clinical effectiveness of autogenic drainage in people with cystic fibrosis with other physiotherapy airway clearance techniques. Search methodsWe searched the Cochrane Cystic Fibrosis Trials Register, compiled from electronic database searches and handsearching of journals and conference abstract books. We also searched the reference lists of relevant articles and reviews, as well as two trials registers (31 August 2017).Dtae of most recent search of the Cochrane Cystic Fibrosis Trials Register: 25 September 2017. Selection criteriaWe identified randomised and quasi-randomised controlled studies comparing autogenic drainage to another airway clearance technique or no therapy in people with cystic fibrosis for at least two treatment sessions. Data collection and analysisData extraction and assessments of risk of bias were independently performed by two authors. The authors assessed the quality of the evidence using the GRADE system. The authors contacted two investigators for further information pertinent to their published studies.
