The Ideal Therapy Must Be Def ined in Each Geographical Area: Experience with a Quadruple Therapy in Spain

Abstract: Quadruple therapy with omeprazole, tetracycline, bismuth subcitrate and metronidazole achieves healing rates up to 95-100%. The 87.7% eradication rate obtained suggests that the regimen we used is a reasonable therapeutic alternative in our area.

“…The most commonly found quadruple regimen contained metronidazole, tetracycline, a gastric acid inhibitor, and a bismuth compound (GBMT) (Table 1). The GBMT regimen was used in 51 studies with a total of 3991 subjects and 85 treatment arms in five continents and 17 countries around the world including Canada, the United States, Russia, Hong Kong, Australia, India, New Zealand, Colombia, and nine countries in Europe 6–8, 10–12, 23–67 . The majority of the treatment arms used a PPI for gastric acid inhibition (85%), bismuth subsalicylate (40%) or colloidal bismuth subcitrate (36%), and came from articles published in peer‐reviewed journals (87%).…”

“…Data from randomized, controlled studies are very limited, to our knowledge. Quadruple therapy based on a combination of tetracycline, bismuth subcitrate, metronidazole, and a PPI has been proposed by experts as a reasonable choice in such patients, based on high efficacy when used as first‐line therapy [1,3–5]. However, few published studies do show data regarding the efficacy of this therapy as second‐line therapy [6].…”

“…One week quadruple therapy of proton pump inhibitor + metronidazole + tetracycline + bismuth subcitrate was frequently studied, with the H. pylori eradication rate ranging from 86% to 100% 14 –18 . The eradication rate was similar if the same treatment was given for 4 days, but poor if the duration was further reduced to 2 days 14 , .…”
