The IASLC Lung Cancer Staging Project: Proposals for Revision of the TNM Stage Groupings in the Forthcoming (Eighth) Edition of the TNM Classification for Lung Cancer

Abstract: The IASLC Staging and Prognostic Factors Committee has collected a new database of 94,708 cases donated from 35 sources in 16 countries around the globe. This has now been analysed by our statistical partners at Cancer Research And Biostatistics and, in close collaboration with the members of the committee proposals have been developed for the T, N, and M categories of the 8th edition of the TNM Classification for lung cancer due to be published late 2016. In this publication we describe the methods used to ev… Show more

“…Both the Danish -and the present populations are taken care of by national health services who provide universal, tax-supported health care, guaranteeing unconstrained access to general practitioners and hospitals. Further, the overall survival in our study was in line with estimates from the International Association for the Study of Lung Cancer [80]. These nearly identical findings may to some degree support and confirm the external validity of our results.…”

“…The current eighth edition of NSCLC regarded LNs staging as a creditable prognostic factor, and LNs staging is based on the location of invaded LNs regardless of the number of PLNs 12. However, the updated pathological TNM stage of colorectal and breast cancer had deemed PLNs as novel criteria 13.…”

“…In conclusion: (1) patient survival after wedge/ sublobar resection of stage I NSCLC is improving, and is not significantly different globally for peripheral small-sized tumors; (2) there are phase III studies comparing lobectomy and segmentectomy but not wedge resection; (3) survival probability of wedge resection seems to be similar to that of SBRT, but SBRT has limitations such as "no-fly-zone", irradiation of hilar tumors and associated complications such as pulmonary fibrosis and hemoptysis; and (4) a suitable subgroup of patients for wedge/sublobar resection may be found based on tumor size, location, margindistance, M/T ratio, and margin cytology.…”

“…[1] Because stage I lung cancer is a localized disease without evidence of metastases [2] and is associated with low morbidity and mortality, minimally invasive (mini-invasive) local therapies are central to managing the high-risk or medically unfit patent such as the elderly. The various types of mini-invasive local therapies for stage I lung cancer are defined by the amount of lung tissue targeted or resected.…”
