Objective To analyze the prevalence of distal caries in mandibular second molars (M2Ms) and its correlation with the threedimensional position of mandibular third molars (M3Ms) by cone beam computed tomography (CBCT) images. Materials and methods CBCT scans involving 421 M3Ms were assessed. The presence of distal caries of M2Ms, patient age and gender, impaction depths and mesial angulation of M3Ms, the cementoenamel junction (CEJ) distances and contact point localization, and the horizontal positions of M3Ms were assessed. Risk factors were identified by multivariate logistic regression analysis. Results The overall prevalence of distal caries in M2Ms was 31.6%. Mesial angulation (16°-75°) of M3Ms, contact point localization at or below the CEJ of M2Ms, and a buccal or lingual position of M3Ms relative to the molar line were identified as risk factors for the prevalence of distal caries in M2Ms (p < 0.05). Distal caries was more severe when the mesial angulation of M3Ms was 16°-75°(p < 0.05). Conclusions Buccal or lingual position of M3Ms may represent a new risk factor for the distal caries in M2Ms. Mesial angulation (16°-75°) of M3Ms is a predictive parameter for both the presence and severity of distal caries in M2Ms. Clinical relevance As the presence of distal caries in M2Ms is significantly associated with the three-dimensional position of M3Ms, watchful monitoring or prophylactic removal of M3Ms should be deliberated when M3Ms are mesially angulated (16°-75°), buccally or lingually positioned, and with the contact point localization at or below the CEJ of M2Ms.

Introdução: No contexto da cirurgia bucomaxilofacial, a exodontia de terceiros molares é um dos procedimentos mais comuns e diversas são as suas indicações. Sua realização pode causar diversas complicações pós-operatórias, como lesão no nervo alveolar inferior (NAI). Para evitar essas complicações, os exames de imagem pré-operatórios se tornam indispensáveis para um procedimento de sucesso. Objetivo: Apresentar, por meio de um relato de caso, a tomografia computadorizada de feixe cônico (TCFC) como exame complementar norteador em exodontia de terceiro molar semi-incluso e impactado próximo ao canal mandibular. Relato do caso: Paciente do sexo feminino, 20 anos de idade, apresentou-se para exodontia de terceiros molares com finalidade ortodôntica. Na análise radiográfica inicial, o dente 38 apresentava-se semi-incluso e impactado em mesio-inclinação, próximo ao dente 37 e com íntima relação com ocanal mandibular. A TCFC revelou que o trajeto do canal mandibular passava por vestibular das raízes do 38 e havia compartilhamento da cortical do canal com a cortical alveolar em ambas as raízes. Após as exodontias, foram feitas as suturas e prescrição de antibiótico e antiinflamatório, além de recomendações pós-operatórias para a paciente. Decorridos oito dias após a cirurgia, a paciente retornou para remoção das suturas, sem apresentar queixas em relação ao pós-operatório. Conclusão: O uso de exames de imagem, como a radiografia panorâmica e a TCFC, permitiu a visualização completa e precisa das estruturas anatômicas para que assim o procedimento fosse concluído sem implicações trans e pós-operatórias negativas.Descritores: Cirurgia Bucal; Dente Impactado; Tomografia Computadorizada de Feixe Cônico.

Mandibular canal vs. inferior alveolar canal: Evidence‐based terminology analysis

Introduction The mandibular canal, as it was formerly named in Terminologia Anatomica (TA), has also been called the inferior alveolar (nerve) canal in many scientific publications. This study was conducted to investigate how these terms have been understood in different regions and different areas of expertise and to discuss the appropriate future application of the term “mandibular canal.” Methods A literature search was conducted using PubMed, and articles using different terms for this structure were classified into two groups, inferior alveolar canal/inferior alveolar nerve canal (IAC/IANC) and the mandibular canal (MC). The 50 most recent articles in each group were included. Publication year, journal title, country of the first author, and affiliation of all authors were recorded in both groups for all 100 articles. Results There was a significant difference between the IAC/IANC and MC groups in the numbers of anatomy journals, other journals, and anatomy affiliations. Turkey published most frequently with a total of 15 articles, followed by Iran with 10 articles, and China/India/United States with seven each. When the six countries of the first author that had three or more publications in each group were compared, only Turkey appeared in both groups; otherwise, different countries were in the two groups. Conclusions Based on the results of this analysis, and considering that the tentative new term “inferior alveolar foramen” is used in the latest TA, we suggest that the mandibular canal should be renamed the “inferior alveolar canal.”
