The HEART Pathway Randomized Trial

Abstract: Background The HEART Pathway is a decision aid designed to identify emergency department patients with acute chest pain for early discharge. No randomized trials have compared the HEART Pathway with usual care. Methods and Results Adult emergency department patients with symptoms related to acute coronary syndrome without ST-elevation on ECG (n=282) were randomized to the HEART Pathway or usual care. In the HEART Pathway arm, emergency department providers used the HEART score, a validated decision aid, and … Show more

“…There has now been a small RCT using a modified version of the HEART score that used structured criteria to categorize history and excludes patients from being classified as low risk if there is a positive troponin result from blood sampling at 0 and 3 hours from arrival at the ED. 70 In this study, the prevalence of major adverse cardiac events was 6%, and the sensitivity for major adverse cardiac events for 141 patients randomized to the modified HEART pathway was 100%.…”

The Australian state of Queensland and the country of New Zealand, with combined populations of nearly 10 million people, now have fully adopted accelerated diagnostic pathways.…”

“…Mahler et al added a repeated troponin measurement at 3 hours to the HEART score (HEART Pathway) to increase the NPV. They were able to identify 39.7% of patients for early discharge (HEART score 0–3 and negative serial troponins) with an NPV of 100% and a PPV of 10.7% 33. The drawback of this approach is that it requires serial troponin testing.…”

“…ESCALA HEART (14,15,16,17) ESCALA HEART PARA DOLOR TORÁCICO La escala HEART (por sus siglas en inglés: History, EKG, Age, Risk factors, Troponin) ha sido validada por varios estudios obteniendo una sensibilidad y especificidad de acuerdo a su puntaje. Sirve para calcular la probabilidad de padecer de enfermedad coronaria.…”
