Introdução: Nos últimos anos, a doença renal crônica (DRC) vem aumentando substancialmente. Com isso, aumentou o número de pacientes na fila de espera para um transplante. Dessa forma, faz-se necessário que os enfermeiros estejam capacitados para prestar assistência aos pacientes desde a fase pré-transplante até a alta hospitalar e o seguimento ambulatorial. Objetivo: Avaliar as evidências disponíveis na literatura sobre orientações de enfermagem para pacientes transplantados renais. Método: Trata-se de uma revisão integrativa da literatura, realizada no período de março a outubro de 2019. A busca e coleta dos dados foram realizadas através do acesso on-line ao Portal CAPES e BVS, utilizando-se descritores em Ciência da Saúde (Decs): Transplante renal, Normas e Enfermagem. Utilizou-se critério de inclusão, artigos na íntegra, na língua portuguesa, inglesa e espanhola, publicados no período de 2014 a 2018. Resultados: Dentre os 344 artigos selecionados, 331 foram eliminados por não atenderem ao objetivo e aos critérios de inclusão; assim, analisaram-se 13 artigos. Conclusão: O estudo mostrou a importância do enfermeiro no cuidado direto na assistência a pacientes transplantados renais, orientando-os quanto à importância do tratamento imunossupressor, aos cuidados com a alimentação e hábitos de vida saudáveis, na intenção de proporcionar maior qualidade de vida e evitando perda do enxerto e até morte do paciente.

BACKGROUND: Many kidney transplant recipients lack the knowledge, abilities, and support they need for self-care. On the other hand, most kidney transplant centers do not have a well-planned and specific training program for them, and educational interventions for kidney transplant recipients have not been adequately effective. This study aimed to describe strategies for improving patient education in kidney transplantation. MATERIALS AND METHODS: Data were collected through semi-structured individual and group interviews with 24 patients, family members, and health-care staff in one of the main kidney transplant centers in Tehran. Participants were selected purposefully, and qualitative content analysis was used to analyze the data. RESULTS: The main finding emerged from the data was the shift from current patient education program to patient- and family-centered education (PFCE). The strategies to achieve this goal were categorized into four main categories including “continuous patient and family education” (pre- and posttransplant patient education), “facilitating the process” (using new technologies, teamwork education, and patient and family accessibility), “strengthening human resources” (empowerment health-care team, allocation of human resources, promoting staffs' motivation, and updating educational content and materials), and “monitoring and evaluation” (correcting patient education recording, supervising the patient education, and appropriate educational evaluation). CONCLUSIONS: Transforming from the current patient education program to PFCE seems to be essential to increase the effectiveness of patient education in kidney transplant process. To this end, providing continuous patient and family education, facilitating the processes, strengthening human resources, and monitoring and evaluation in health-care organizations conducting the kidney transplantation is necessary.

Pretransplant Patient Education in Solid-organ Transplant: A Narrative Review

Education for pretransplant, solid-organ recipient candidates aims to improve knowledge and understanding about the transplant process, outcomes, and potential complications to support informed, shared decision-making to reduce fears and anxieties about transplant, inform expectations, and facilitate adjustment to posttransplant life. In this review, we summarize novel pretransplant initiatives and approaches to educate solid-organ transplant recipient candidates. First, we review approaches that may be common to all solid-organ transplants, then we summarize interventions specific to kidney, liver, lung, and heart transplant. We describe evidence that emphasizes the need for multidisciplinary approaches to transplant education. We also summarize initiatives that consider online (eHealth) and mobile (mHealth) solutions. Finally, we highlight education initiatives that support racialized or otherwise marginalized communities to improve equitable access to solid-organ transplant. A considerable amount of work has been done in solid-organ transplant since the early 2000s with promising results. However, many studies on education for pretransplant recipient candidates involve relatively small samples and nonrandomized designs and focus on short-term surrogate outcomes. Overall, many of these studies have a high risk of bias. Frequently, interventions assessed are not well characterized or they are combined with administrative and data-driven initiatives into multifaceted interventions, which makes it difficult to assess the impact of the education component on outcomes. In the future, well-designed studies rigorously assessing well-defined surrogate and clinical outcomes will be needed to evaluate the impact of many promising initiatives.
