In modern conditions, valvular heart diseases (VHD) are one of the most common pathologies among cardiovascular diseases with a dynamic change in the phenotype of patients. An increase in the prevalence of VHD is currently observed due to the active implementation of diagnostic methods in cardiology. Geographical differences in the genesis of the development of valvular heart defects are noted, and the portrait of patients also changes as a result of aging and the addition of comorbid pathology. The purpose of the literature review was to present current trends in changing phenotype of patients with VHD, to study current data on the epidemiology of valve pathology, the contribution of various cardiovascular risk factors and comorbidity of patients on the course of the disease. Current data on the number of surgical interventions performed for VHD based on Russian, European, American, Australian and other studies, data on patient survival and mortality, as well as differences in these indicators in age groups of different countries are presented. The review will be useful for doctors to understand the modern portrait of a patient with VHD, trends in cardiovascular risk factors that influence the course of the disease and prognosis in patients with VHD.

IntroductionThe prevalence of valvular heart diseases remain considerably high in Africa, largely but not solely due to rheumatic heart disease. Valvular heart surgeries have emerged as the cornerstone in their management. While several studies have reported data on outcomes following heart valve surgery in many developed countries, there is a staggering paucity of data and evidence reporting the outcomes in the Africa population. The aim of this study is to report the perioperative outcomes following valvular heart surgery in Africa.MethodsThe Preferred Reporting Items for Systematic Reviews and Meta‐analysis guideline was utilized. Electronic searches were performed using PubMed, African journal online, and Research gate from inception to June 2023. The primary endpoints were overall mortality and 30‐day mortality, and secondary endpoints included postoperative complications, length of hospital, and intensive care stays. The outcome data were pooled together and analyzed with the random effect model for proportions and mean for meta‐analysis using the R software.ResultsThis systematic review identified 31 studies that fulfilled the study eligibility criteria and all were observational studies. The countries in which these studies were carried out include South Africa, Ethiopia, Egypt, Mali, Rwanda, Nigeria, Cameroon, Ghana, Senegal, Tanzania, and Kenya. Statistical analysis reported a pooled overall mortality of 10.48% and a pooled 30‐day mortality of 4.59%.ConclusionSeveral obstacles, such as lack of financial resources and inadequate infrastructure, continue to impede valvular heart surgery practice in many parts of Africa. Future studies need to focus on identifying factors associated with this poor early mortality.

Background: Valvular heart disease and pulmonary hypertension (PH) are two distinct but often interconnected cardiac conditions that can affect children and encompass abnormalities of the heart's valves such as the aortic, mitral, tricuspid, and pulmonary valves, and can present with symptoms such as chest pain, fatigue, and dyspnea. In contrast, PH refers to elevated blood pressure in the pulmonary arteries. Routine screening for these conditions is needed for prompt diagnosis and management. This study was conducted to obtain data on children with valvular heart disease or PH who participated in the Community Service Program of the Department of Cardiology and Vascular Disease, Universitas Sumatera Utara. Method: This study was a descriptive study conducted through a cross-sectional study design. Data was collected from interviews and on-the-spot examination with validated measurement tools. Data was analyzed using SPSS version 26. Categorical variables were presented using frequency (n) and percentage (%), and numerical variables with normally distributed data were presented with mean and standard deviation (SD). In non-normally distributed data, numerical variables were presented using the median and interquartile range. Results: There were 157 children included in this study. Most subjects were female (n=94, 59.9%) in the age range 12 to 16 years old. Most subjects (n=152, 96.8%) had normal mitral valves, three subjects had anterior mitral valve prolapse (1.9%), one subject (0.6%) had anterior mitral valve thickening and one subject (0.6%) had mild mitral regurgitation. 10 subjects (5.7%) had pulmonary regurgitation. Two subjects had tricuspid regurgitation (1.2%). No subjects had any aortic abnormalities. No subjects had a PH. Conclusion: In the children population in Rantau-Prapat City, most subjects with valve abnormalities had mitral and pulmonary valve abnormalities. No subjects had aortic valve abnormalities or PH.
