“…Aphakia is also a known risk factor for failure in adult trabeculectomy surgery. 25,26 Previous reports of outcome of filtering surgery without the use of any adjunctive antifibrotic agents in the management of a paediatric glaucoma have been disappointing. [9][10][11] The use of antifibroproliferative agents such as 5-fluorouracil (5-FU) 27,28 and MMC, 29,30 which may improve the surgical prognosis, has been studied extensively in adults.…”

“…The most common cause of the failure of this surgery is scarring secondary to fibroblast proliferation and fibrosis at the episcleral conjunctival interface [1]. The risk factors which may influence the outcome of filtering surgery include aphakia [2]and youth [3], ocular inflammation and neovascular glaucoma [4], ethnic origin [5], previous surgery [6], long-term use of topical antiglaucoma medications and possibly diabetes [7, 8, 9]. The mechanism or mechanisms by which these factors reduce success is not fully understood.…”

“…Failure was secondary to external conjunctival bleb failure in 10 eyes, to internal clo sure of the anterior chamber Supramid tube in 5 eyes, to phthisis bulbi secondary to choroidal hemorrhage in 2 eyes and to external erosion of the Silastic end in l eye. 12 Mortality during long-term follow-up was high in our se ries. The complications of an underlying diabetes mellitus were the most common causes of death (15 out of 22 pa tients).…”

“…External bleb scarring was considered the most common cause of surgical failure [3,4,11,12], In our sample IOP control in the early postoperative peri od proved very difficult. Sixteen out o f 28 patients (57.1%) developed IOPs below the manufacturer's stated lower limit of closing pressure (9 mmHg); the 7 patients who had se rous choroidal effusion belonged to this group.…”
