Categorical public health programs have always set process and outcome objectives for specific diseases and health promotion efforts (5), but Healthy People 2000 (64) and its predecessor report Promoting Health/Preventing Disease: Objectives for the Nationfor 1990 (63) were the first to set detailed objectives for the entire public health system in the United States, Efforts at establishing and achieving the national public health objectives have been well documented by researchers, the lead Public Health Service agencies, and their state counterparts (65-67). However, implementing the objectives at the local level is a complicated task that has not been as well documented.As part of a growing interest in community health planning, Healthy People 2000 and other planning tools have been increasingly used to mobilize communities for health. In this review we will summarize the relevance of national health objectives for community use, other health planning tools available to implement these objectives, and the formation of partnerships between health agencies and community groups.
