The frequency of sicca symptoms and Sjögren's syndrome in patients with systemic sclerosis

Abstract: In the present study, highly frequent sicca symptoms and Sjögren's syndrome based on AECG criteria were noted in patients with systemic sclerosis. The SSc-SS patient group had less severe clinical course and lung involvement.

“…In the setting of salivary gland dysfunction, there is a decrease in salivary nitrate concentration and corollary increase in urinary excretion, thereby reducing total body nitrate content and its enterosalivary circulation[84,193,231,293]. Significantly, several diseases that share both disproportionately high rates of vascular disease and PH, including Sjögren's and systemic sclerosis[294297], HIV[7,11], and cystic fibrosis[298] also feature dysregulated salivary flow[299302] and altered oral microbial populations[224,303306]. While there is no conclusive link between these factors, these associations offer a potentially novel line of investigation.…”

“…In SSc, xerostomia is not only a subjective sensation, but it is usually the consequence of an objective reduction in the quantity of the saliva produced. SSc patient's salivary flow was assessed in different studies (Andonopoulos et al , ; Nagy et al , ; Chu et al , ; Kobak et al , ; Baron et al , ,b). The largest cohort study (163 SSc patients vs 231 controls), performed recently by the Canadian Scleroderma Research Group, revealed that the disease was associated with a lesser saliva production (63.19 mg min −1 vs 147.52 mg min −1 ) (Baron et al , ).…”

“…A classic triad of symptoms predominates in the clinical manifestation of Sjögren's syndrome; aside from xerostomia, it also includes dry eye syndrome/keratoconjuntivitis sicca and polyarthritis [18]. The presence of proteins against glandular duct cells, anti-RO/SSA and antiLa/SSB antibodies and rheumatoid factor leads to the destruction of secretory vesicles with associated lymphocytic infiltration [19].…”
