Introduction:Diclofenac is an over-the-counter (OTC) non-steroidal anti-inflammatory drug (NSAID) that works through non-selective inhibition of the cyclooxygenase (COX) enzyme, thereby inhibiting the release of prostaglandins. Known for its efficacy in alleviating inflammation, pain, and fever symptoms, concerns have arisen regarding the potential toxicity of Diclofenac. Its toxicity can cause a wide variety of adverse effects, ranging from self-limiting side effects that disappear after treatment cessation to an increased risk of death due to cardiovascular diseases and strokes. Determining the threshold dose that distinguishes between mild, self-limiting side effects and more severe adverse effects poses a major challenge in diclofenac toxicity research. Method: This article reviewed various doses and different organ toxicities observed in animal and human studies using diclofenac. Aim: Our objective is to define the toxicities in fourteen organs and organize them into different categories as animal toxicities, human toxicities, and teratogenicity through pathological changes. Conclusion: By searching different databases and collecting the observed toxicities, we have concluded that the primary affected organs are the stomach, liver, and kidney. However, there is limited knowledge on the toxic effects on the brain, heart, lungs, spleen, eye, bone, bone marrow and blood, and testes, while rare available data for the pancreas, bladder, skin, muscle, and uterus.

Aim. Description of risk factors, pathogenesis and treatment strategies of gastrointestinal bleeding (GIB) in the course of antithrombotic therapy in patients with coronary heart disease (CHD).Key points. Risk factors of GIB during antithrombotic therapy in CHD patients include: GIB, gastric and/or duodenal ulcer in the history, reflux esophagitis, presence of H. pylori, inflammatory bowel disease, diverticula, haemorrhoids, angiodysplasia, gastrointestinal neoplasia, age above 65 years, concomitant treatment with non-steroidal anti-inflammatory drugs (NSAIDs), glomerular filtration rate <50 mL/min, high doses of direct oral anticoagulants (DOACs) ≥3 in HAS-BLED score. Pathogenesis of GIB in the course of antithrombotic therapy is associated with systemic hypocoagulation and local damaging effects of these drugs. Strategies of GIB treatment during DOAC therapy in patients with CHD are determined by severity of bleeding and threat to life. Aside to standard conservative measures, endoscopic or surgical haemostasis requires usage of antidotes to suppress effects of DOACs and other specific drugs in severe cases.Conclusion. GIB associated with antithrombotic therapy in CHD patients poses a serious medical problem of growing importance with wider application of anticoagulant drugs. Antithrombotic therapy requires accurate decision making, risk assessment, careful monitoring of the patient’s condition and timely diagnosis of gastrointestinal disorders following good rationale in GIB prevention.

Extensive use of antithrombotic drugs (ATD) in patients with ischemic heart disease (IHD), on the one hand, provides a considerable decrease in the risk for development of life-threatening cardiovascular complications but on the other hand, is associated with a risk of gastrointestinal bleedings (GIB), which may develop in 0.5-1.0 % of patients. In such cases, the major measures for prevention of GIB are strict adherence to indications for the ATD treatment, detection and analysis of risk factors for GIB and their elimination as far as feasible. For evaluation of GIB risk in patients with IHD, the PRECISE-DAPT and DAPT, HAS-BLED scales should be used. If the risk factors are non-modifiable the therapeutic tactics for further management of these patients should be strictly individual with determining the nature of damage, degree of a risk for present and possible complications, and the range of required therapeutic and diagnostic measures. The use of ATD requires monitoring of the patient’s condition to timely detect and treat GI complications.
