The Findings of Electrocardiography in Patients with Cardiac Amyloidosis

Abstract: In CA patients, low voltage on limb leads and pseudo-infarct pattern were the most common ECG findings. Atrial arrhythmia and atrioventricular block were the most common arrhythmias in CA patients. The combination of low voltage on limb leads and pseudo-infarct pattern had high specificity and positive predictive value for the diagnosis of CA.

“…A QS pattern in at least two precordial chest leads ("a pseudo-infarct pattern") and an inversion or depression of the T or ST section in the lateral chest leads can be seen in some patients [35][36][37][38][39] . The Holter ECG shows episodes of ventricular and supraventricular arrhythmia in a large number of asymptomatic patients.…”

“…However, echocardiography cannot confirm diagnosis in isolation and is also unable to distinguish between various types of amyloidosis. A finding typical for infiltrative affection of the myocardium is end-diastolic thickness of the interventricular septum or left ventricular wall > 12 mm 35 . The use of the term "hypertrophy" is, in this case, incorrect, as the condition is not caused by the hypertrophy of cardiomyocytes, but by amyloid deposition.…”

“…However, this is thought to be the result of signficant under diagnosis and as improved non-invasive diagnostic approaches arise, this number is likely to increase [3]. In one recent analysis, ATTRwt occurs in greater than 25% of individuals at autopsy but the clinical significance of this finding remains unclear [4,5]. In an often cited but likely regionally, ethnic diversity limited survey, the age-adjusted incidence of AL amyloidosis is estimated to be beetween 5.1 and 12.8 per million person years [6].…”

“…As the heart becomes progressively larger and as the amyloid deposits into the heart's conduction system, patients are prone to arrythmias, especially atrial fibrillation [1,5]. Sudden cardiac death, often from ventricular and PEA rhythms, have also been associated [3].…”
