“…Knowing that what is recommended in the actions programmed for the elderly has a real possibility of being integrated through the PHE, (1) it is not possible to develop the actions planned for the care of the elderly without incorporating the FHSC teams, especially because one of the priority roles of the FHSC is technical and pedagogical support in order to address the culture of care fragmentation and the lack of accountability for care.
