The Failure of Pyridoxine in Suppression of Puerperal Lactation

Abstract: Summary The benefits of pyridoxine in the suppression of lactation were assessed in a double blind controlled trial on 175 puerperal women. No significant differences were demonstrated between pyridoxine and the placebo whether assessed by subjective discomfort or by the persistence of lactation.

“…The method of outcome assessment varied among the studies. Two studies assessed the effect of high‐dose pyridoxine use on prolactin levels, whereas pyridoxine efficacy was evaluated clinically in six of the studies . However, different parameters were used to describe clinical efficacy, including breast discomfort, breast consistency, breast engorgement and cessation of lactation.…”

“…Overall, the total number of patients who received pyridoxine was not very large (n=471 patients), and sample size calculations were not presented in any of the included studies. Furthermore, the specific study designs were not clearly defined in the non‐randomized studies, and all of the studies included in the current review had been published for more than 35 years ago . Based on these observations, the strength of the current evidence is not sufficient to recommend the routine use of pyridoxine for post‐partum lactation inhibition in clinical practice.…”

“…Oestrogens, either alone or in combination with androgens, have been widely used for this purpose. Pyridoxine in high doses has been tried but the results have been largely disappointing (6). Also alarming is the increased risk of associated thromboembolism (5).…”
