The Experience of Chronic Illness Transitional Care: A Qualitative Systematic Review

Abstract: This is a qualitative systematic review of recent qualitative studies of the experiences and perceptions of both individuals with chronic illness(es) and their caregivers regarding hospital-to-home transitions.
