The Expansion of Gastrointestinal-associated αβ T Cell Clones in Peripheral Blood Over Time Is a Disease Feature of Severe Acute Graft-Versus-Host Disease

Abstract: 228 Donor T cell alloreactivity in hematopoietic stem cell transplantation (HSCT) can result in acute graft-versus-host disease (GVHD) which is a major cause of morbidity and mortality that limits the clinical use of HSCT. Despite the potential large diversity in possible T cells (1015) it is thought that within each individual patient undergoing HSCT, a tractable and defined number of T cell clones mediate GVHD. We hypothesized that the identification of the most frequent T cell clones in the g… Show more

“…Alternatively, detailed study of endoscopic tissue and/or blood with new molecular techniques such as mRNA or T cell receptor profiling of the biopsy tissue sites could improve diagnostic yield, and even potentially stratify patients into those who will respond to steroids vs. those who will not. In addition, we did not examine patients for other causes of their symptoms after a negative endoscopic evaluation. For example, in patients with diarrhoea, other aetiologies including small bowel bacterial overgrowth, steatorrhea from pancreatic insufficiency or other infectious causes might be in the differential diagnosis, and an upper endoscopy with duodenal aspiration would be useful for further evaluation.…”
