The Evolution of Some European Diagnostic Concepts Relevant to the Category of Schizoaffective Psychoses

Abstract: The evolution of the German-Scandinavian concept of cycloid psychosis and of the French concept of ‘bouffée délirante’ is analyzed. It is stressed that they probably correspond to the same clinical entity, not overlapping with any of the American nosological categories, although sharing a number of features with the original Kasanin’s picture of schizoaffective psychosis. Moreover, a survey is given of the evidence supporting the construct and predictive validity of the concept of cycloid psychosis.

“…Nosology As Maj [9] pointed out, the evolution of the concept of schizoaffective disorders is a reflection of the changes of the various concepts of schizophrenia and mood disorders over time. Schizoaffective disorders are considered a special variant of schizophrenia, a variant of mood disorders, an independent nosologic entity, a psychotic continuum between schizophrenia and mood disorders, a result of a comorbidity of schizophrenia with mood disorders, and an inhomogeneous group of psychoses.…”

“…In addition to this diagnostic dichotomy, many authors have described other groups of non-organic psychiatric patients and have claimed them as independent nosological groups. Thus, the concept of cycloid psychosis (1) has been used by many psychiatrists (2,3 ) to denote a specific group of syndromes that often are difficult to distinguish from the 2 major psychoses in that the clinical profile is polymorphous with a combination of manic-depressive and schizophrenic features. The psychotic manifestation, which often is preceded by a period of sleep disturbances, is characterized by an acute onset, with a rapid change from a state of health to a full-blown psychosis, often within a few hours or a few days.…”

“…This condition, at the extreme psychotic boundary of the bipolar spectrum, continues to challenge nosologists [38]. It overlaps with the French concept of 'bouffée délirante' [39] and the Scandinavian rubric of 'reactive psychosis' [40][41][42][43][44].…”
