The Evidence for Prosthodontic Treatment Planning for Older, Partially Dentate Patients

Abstract: This paper provides a review of the available evidence for traditional prosthodontic need determination and management strategies for older, partially dentate individuals, and compares it to the documented functional benefits of a more limited yet targeted approach to alleviating morphological shortfalls for this group of people, at the population level. The reasons for which the traditional total replacement strategy is not universally applicable or viable are discussed, and research is presented which tests … Show more

“…Yet, the scientifi c evidence for the validity of a number of such normative measures as the importance of arch integrity to occlusal stability, chewing ability and prevention of temporomandibular disorders is at best tenuous [25] . “…However, for social, dental or fi nancial reasons, this might neither be attainable nor indeed necessary [27] . Because prosthodontic therapy is intrinsically expensive with an inherent biological price and a fi nite life-span, it is increasingly recognized that a perspective broader than the normative one is required, specifi cally one that includes patients' perceptions, attitudes and expectations regarding the planned treatment [6,25] . How this web of factors interrelates to permit alternative solutions for partial edentulism remains to be explored [26] .…”

“…The removable dental prostheses are also classified into removable partial denture and complete denture [3]. Based on previous studies, social and geographical variations in prosthetic replacement may be related to differences in both patients' and dentists' attitudes towards oral health as well as socioeconomic status [13, 1719]. …”

“…Differences in clinical decision-making affect the cost-effectiveness and costbenefit of oral healthcare, with impact on both the individual patient as well as at the population level [7,10] and yet they remain poorly understood [11] . Indeed, there is a perception that clinical decisions tend to be made intuitively, even idiosyncratically, and that dentists do not share a common decision-making process [12] .…”
