Purpose: To evaluate and compare changes in retinal nerve fiber layer (RNFL) thickness in patients with obstructive sleep apnea/hypopnea syndrome (OSAHS). Methods: The Cochrane Library, Medline, and Embase were screened using our key words. Results were carefully reviewed to ensure that the included studies met the inclusion/exclusion criteria, and the quality of the studies was assessed using the Newcastle-Ottawa Scale. All included studies categorized patients with OSAHS into 3 groups (mild, moderate, and severe), and measured average and 4-quadrant (temporal, superior, nasal, and inferior) RNFL thickness. All studies included a healthy control group. The weighted mean differences and 95% confidence intervals were calculated for the continuous outcomes. Results: Ten case-control studies were included in the meta-analysis, consisting of a total of 811 OSAHS group and 868 healthy eyes. A meta-analysis of the data showed that the average RNFL thicknesses in the mild, moderate, and severe OSAHS groups were significantly decreased compared to healthy controls. Additionally, RNFL thickness was significantly reduced in all but the temporal quadrant in the moderate and severe OSAHS groups when compared to healthy controls. Conclusions: On the basis of these results, we suggest that peripapillary RNFL thickness as measured by optical coherence tomography could be a useful tool to monitor and assess the severity of OSAHS in patients. Further studies are required in order to differentiate these RNFL changes from glaucomatous changes. This has not been properly examined in any of the studies we were able to identify.

Objective: To evaluate retinal nerve fiber layer thickness (RNLF) with optical coherence tomography (OCT) and optic nerve function with visual evoked potential (VEP) in patients with obstructive sleep apnea syndrome (OSAS). Methods:Fifty-one eyes of 30 newly diagnosed OSAS patients who received no treatment were included. The RNFL analysis with OCT and pattern VEP test (120',30',15',7' pattern size) was performed to all patients. Results:Of 30 patients, 20 were females and 10 were males. None of the patients were obese with a body mass index of (BMI) < 30 kg/m 2 . The mean Apnea Index of the patients was 42.5 ± 24.3. All patients had severe OSAS with an Apne Index of ≥ 30. Visual acuity was 20/20, intraocular pressure and optic disc were within natural limits in the all of the eyes included in the study. A statistically significant decrease (p < 0.05) was detected in the all parameters of p100 wave amplitude of VEP tests. Compared to the standard data of the healthy individuals at the same age, there was no significant difference in the p100 latency. Although there was thinning of the RNFL thickness, there was no statistically significant difference (p > 0.05). Conclusion:In patients with OSAS, ischemia during deep sleep causes optic nerve dysfunction which can be detected by electrophysiological test, even in asymptomatic patients. In this study, the importance of early diagnosis and treatment is emphasized, and particularly early treatment may halt the progression of the optic nerve dysfunction. For a better understanding of these diseases and ocular manifestations, further long-term studies are needed.

Tüm katılımcıların tanısı göğüs hastalıkları kliniği uyku laboratuvarında polisomnografi yapılarak konulmuştur. Hastaların rutin göz muayenelerinin ardından kornea topografisi ile ön segment ölçümleri yapılmıştır. Grupların merkezi kornea kalınlığı (MKK), dik ve düz keratometri değerleri, ön kamara derinliği (ÖKD), ön kamara hacmi (ÖKH), ön kamara açısı (ÖKA), kornea hacmi (KH), görülebilir horizontal iris çapı (GHİÇ) ve horizontal ön kamara çapı (HÖKÇ) verileri karşılaştırılmıştır. Ortalama apne hipopne indeksi (AHI) değeri OUAS grubunda 26,78±23,56 iken, kontrol grubunda 3,00±1,64 idi (p<0,001).
