Background: Polymorphous adenocarcinoma (PAC) is the second-most common malignant tumour of the minor salivary glands. Although PAC predominantly affects the palate, it can also involve the buccal mucosa. This systematic review aims to investigate the literature data about PAC. Furthermore, we report two cases of patients affected by PAC in an infrequently considered anatomical site. Methods: According to PRISMA guidelines, a systematic review search was conducted on PubMed, Scopus, and Web of Science. Observational studies conducted on patients with a histological diagnosis of PAC were selected and analysed. Furthermore, two cases of patients with PAC affecting the buccal mucosa were reported. Results: Twenty-nine studies were included, and 143 patients affected by PAC were analysed (62 males, 75 females, and 6 undefined, with a mean age of 57.4 ± 14.5 years). The palate was the most affected site (99/143, 69.2%), followed by the buccal mucosa (12/143, 8.4%). Moreover, we report two cases of patients with PAC affecting the buccal mucosa (one male and one female, with a mean age of 70.5 ± 2.5 years). Conclusions: The present study underscores the importance of considering the buccal mucosa as a possible location of minor salivary gland tumours; although it is a less-considered affliction, it is not uncommon.

We aimed to validate the diagnostic accuracy of a novel sonographic scoring model and compare it with other methods in the evaluation of malignant major salivary gland tumors. We enrolled 138 patients who received neck ultrasound (US) with fine needle aspiration (FNA) and further operations or core needle biopsies for major salivary gland tumors from June 2015 to October 2021. The sonographic scoring model was presented as 2.08 × (vague boundary) + 1.75 × (regional lymphadenopathy) + 1.18 × (irregular or speculated shape) + 1.45 × (absence of posterior acoustic enhancement) + 2.4 × (calcification). We compared the diagnostic accuracy of the sonographic scoring model with shear wave elastography (SWE), real-time elastography (RTE), and US-FNA cytology for differentiating between benign and malignant lesions. The sensitivity, specificity, and accuracy were 58%, 89%, and 85% for the sonographic scoring model, 74%, 62%, and 64% for the SD of SWE with optimal cutoff value of 31.5 kPa, 69%, 70%, and 70% for the 4-point scoring system of RTE, and 74%, 93%, and 91% for US-FNA cytology, respectively. The sonographic scoring model is feasible as assistance in the evaluation of major salivary gland tumors. US-FNA cytology remains the tool of choice in diagnosing malignant salivary gland tumors.

Objective: The aim of this study was to quantify the association between the incidence and mortality of salivary gland cancer (SGC) with the consumption of tobacco and with the Human Development Index (HDI). Material and Methods: An ecological study was designed using SGC incidence and mortality data in 172 countries in 2020, reported per 100,000 population with an age range of 10–85 years. Such dependent variables were obtained from the Global Cancer Observatory (GLOBOCAN). The prevalences of smoked and chewed tobacco were obtained from reports made in 2019, sourced from Global Burden of Diseases Study, while the HDI was obtained from the Human Development Report (2019) presented by the United Nations Development Program, and were considered independent variables. Statistical analysis was performed in Stata using Spearman’s correlation test. Results: In 2020, there were 53,583 new cases of SGC worldwide, and countries with very high HDI exhibited a higher incidence. In terms of mortality, there were 22,778 cases worldwide, and countries with a moderate HDI showed a higher number of deaths. SGC mortality was associated with chewing tobacco use overall and in women. Surprising negative associations were found with the incidence of SGC and tobacco smoking in men, as well as with mortality of SGC and smoking overall and in women. CGS mortality was negatively associated with the HDI and all its components. Conclusion: In this international overview, SGC mortality is associated with chewing tobacco in women, as well as with the HDI and its components.
