The era of centers: the influence of establishing specialized centers on patients’ choice of hospital

Abstract: Physicians are the most important source of recommendations for oncological patients. From the marketing point of view, intensive involvement of local private-practice physicians is necessary. The availability of certified perinatal centers does not currently play any part in patients' choice of hospital.

“…In a survey including 1,073 patients in a department of gynaecology in Germany conducted in 2007, 65.6% of the patients were found to be familiar with certified centres [34]. In a further survey including 2,389 patients contacted between 2007 and 2008, the availability of a centre or the presence of certification, respectively, represented the major reason for recommending the hospital or for presenting there again in 34.3% and 43.5% of cases [35]. The data available show on the one hand that the perception and appreciation of care in certified structures have substantially increased in recent years, and on the other that a high level of penetration of the idea of establishing and certifying centres has already developed among the patients affected.…”

“…59.4 % of the patients followed the recommendation of their gynaecologist when deciding on a clinic. The importance of referring doctors and, to a lesser extent, certification was also underscored as a selection criterion for patients by Lux and colleagues [5]. This is most probably due to the comparatively high level of awareness of the concept of certified breast cancer centres [6].…”

“…The five-year survival rate rose from 73 % in 1998-1992 to 80% with the initial diagnosis in 1998-2002, presumably due to the earlier detection of tumours while still small and primarily due to improved adjuvant therapy [21]. Further progress is expected by the establishment of certified organ cancer centres [22][23][24], the implementation and consistent use of guidelines [25], the use of targeted therapies [26], early detection via mammography with suitable quality assurance [27] and the identification of new and prognostic markers [28]. This will only be measurable, however, if the implementation of these new structures and innovations is accompanied by health services research-related projects.…”
