Abstract: ARDS has a high mortality in these Chinese PICUs, especially in those with pneumonia and sepsis, and adequate management including lung protective ventilation strategy is required.

“…This fi nding is consistent with the average ALI/ARDS incidence in PICU reported elsewhere. [4][5][6][7][8][9] The reported ARDS prevalence is about three times higher than that of China in 2004, [10] and the relatively higher ALI/ARDS prevalence may be correlated to a higher percentage of severe cases in PICU.…”

“…Since respiratory failure due to ARDS is not just typical of the adult population but it is represented also in infants, AECC definition was clinically used either in adult or in pediatric critical care (10)(11)(12) to contextualize the grade of lung injury. However, oxygenation index (OI), proposed for the first time by Dr. Bartlett studying indications for ECMO in neonates with respiratory failure (13,14), is a commonly accepted indicator to describe lung injury in the pediatric population.…”

“…Then, multivariate Cox's regressions were performed for the same outcomes adjusting for age, sex, PRISM-III 24 , ARDS type (primary/secondary), and study center. These covariates were chosen because of their epidemiological role [4,14] and their association with the outcomes [11,15,16] by unanimous agreement within the working group. BD definition was inserted in the model and considered to have three categories.…”

“…The AECC definition had been applied to pediatric ARDS without specific evaluations or changes [4][5][6]. However, ARDS is a complex syndrome: any definition is an attempt to categorize the clinical reality and this complexity is partially due to the different underlying conditions and triggering diseases across the ages [7].…”
