The entry ECG in the early diagnosis and prognostic stratification of patients with suspected acute myocardial infarction

Abstract: 475 patients with suspected uncomplicated myocardial infarction (MI) were divided into 3 groups based on their entry ECG: group 1--significant ST elevation; group 2a--ST depression or T inversion; group 2b--normal ECG. Infarction was confirmed in 99.7% of group 1, 68.5% of group 2a and 39.7% of group 2b patients. Despite similar clinical, haemodynamic and historical variables at presentation, group 1 patients had significantly larger MI, more in-hospital complications and a higher short-term and long-term mort… Show more

“…Although an important management tool for guiding therapy [11,12], ECG is a poor diagnostic test for AMI as about half of the AMI patients who present to the Emergency Department (ED) show normal or nondiagnostic electrocardiograms, making early diagnosis of AMI difficult [13]. Therefore, measurement of cardiac markers is critical in assisting the diagnosis of AMI and in assessing patients with nondiagnostic ECGs.…”

“…Vários estudos têm sido realizados para determinar a acurácia diagnóstica e a utilidade da história clínica e do ECG em pacientes admitidos na sala de emergência com dor torácica para o diagnóstico de infarto agudo do miocárdio 14,16,[30][31][32][33] . A característica anginosa da dor torácica tem sido identificada como o dado com maior poder preditivo de doença coronariana aguda 15,[34][35][36] .…”

“…The diagnostic accuracy of the ECG is limited; sensitivity varies from 41.0% to 75.3%, depending on the criteria used to define AMI [2,3]. The incidence of disease further limits the value of the ECG.…”

“…Although ST-segment elevation has a high specificity for AMI and has been shown to identify those patients who will respond to thrombolytic therapy, not all patients with ST-segment elevation have AMI, and the specificity in patients receiving thrombolysis is between 94.4% and 99% [2,35]. There is currently no evidence that biochemical markers should be used to select patients for thrombolysis, but cTn measurements should be used as part of the routine assessment of patients with ST-segment elevationsuspected ACS to audit diagnostic accuracy with measurement of cTn at 12 hours from admission.…”
