Introduction : Risk management is a frequent and step-by-step process that helps system managers improve decision-making in times of risk. During this process, the risk of errors is reduced, and Opportunities for improvement are increased. Given the increasing importance of patient safety and the role of risk management in reducing medical errors and treatment costs, this study was conducted to identify the executive framework of active risk management in the operating room of hospitals from the perspective of experts and administrative staff. Method This qualitative phenomenological study was conducted with a targeted sampling method and included 20 experts and executive staff as the study sample. Data were collected using a researcher-developed semi-structured interview and were analyzed using the framework analysis method. Results The results showed that active risk management consists of four components, including types of risks, principles, processes, and risk management framework in the operating room with 27 main themes based on the standard classification for types of risks (the Eight Risk Domains of Enterprise), principles, processes, and framework) principles, framework, and risk management process from ISO 3100( and 115 sub-themes. Conclusion By identifying different types of risks, analyzing and reporting them continuously, implementing the principles and framework of clinical risk management, creating an open and fair safety culture to achieve patient safety, creating programs and policies for error reporting, continuous training, skill development, institutionalizing methods to prevent errors, and requiring employees to implement the risk management process fully, a suitable model of clinical risk management can be implemented in hospitals’ operating rooms.

Although a large amount of accident data in terms of categorical attributes and free texts are available across large enterprises involving high-risk operations, the methodology for analyzing such mixed data is still under development. The present study proposed a new methodological approach to extract useful inherent patterns or rules for accident causation using association rule mining (ARM) of both incident narratives (unstructured texts) and categorical data. Incidents data from an integrated steel plant for a period of four years (2010–2013) are used for model building and analysis. In the first phase, the text mining approach is employed to find out the basic events that could lead to the occurrences of faults or incident events. In the second phase, text-based ARM has been used to extract the useful rules from unstructured texts as well as structured categorical attributes. A total of 23 best item-set rules are extracted. The findings help the management of the plant to augment the cause and effect analysis of accident occurrences as well as quantifying the effects of the causes, which can also be automated to minimize the human involvement.

The operational activities conducted in a shipyard are exposed to high risk associated with human factors. To investigate human factors involved in shipyard operational accidents, a double-nested model was proposed in the present study. The modified human factor analysis classification system (HFACS) was applied to identify the human factors involved in the accidents, the results of which were then converted into diverse components of a fault tree and, as a result, a single-level nested model was established. For the development of a double-nested model, the structured fault tree was mapped into a Bayesian network (BN), which can be simulated with the obtained prior probabilities of parent nodes and the conditional probability table by fuzzy theory and expert elicitation. Finally, the developed BN model is simulated for various scenarios to analyze the identified human factors by means of structural analysis, path dependencies and sensitivity analysis. The general interpretation of these analysis verify the effectiveness of the proposed methodology to evaluate the human factor risks involved in operational accidents in a shipyard.
