The endoscopic ultrasound-assisted Rendez-Vous technique for treatment of recurrent pancreatitis due to pancreas divisum and ansa pancreatica

Abstract: Endoscopic treatment of pancreatic ductal malformations causing recurrent acute pancreatitis, such as pancreas divisum or ansa pancreatica, is mainly based on the sphincterotomy of the minor papilla. However, the technical complexity of conventional endoscopic retrograde cholangiopancreatography (ERCP) is increased in patients presenting anatomical variants like these and it may be unsuccessful. We report the case of a pancreas divisum combined with ansa pancreatica and describe the cannulation and sphincterot… Show more

“…Once ansa pancreatica is diagnosed, the patient may undergo sphincterotomy of the minor papilla to relieve the obstruction, or in severe cases, endoscopic ligation of the ansa deformity may be needed. Cannulation of the minor papilla can be challenging, and in such situations, an ultrasound-assisted rendezvous technique via the ansa has been performed [ 14 ].…”

“…17 In recent years, EUS-guided puncture combined with ERCP has been more widely used in the diagnosis and treatment of pancreatic diseases. [18][19][20][21][22] It was first reported by Dewitt et al as a technique performed in the patient of PD. 23 The procedure is as follows: First, EUS is used to guide a needle through the stomach and into the main pancreatic duct.…”

“…A snare or reticular basket is then used to capture the guide wire to form a EUSassisted rendezvous technique. 22 After rendezvous, the catheter can very easily enter the main pancreatic duct and conduct the corresponding endoscopic treatment. This technique was successfully used on both patients in these case reports.…”
