Aim To explore the impact of dental anxiety on daily living. Method Twenty people attending a dental sedation clinic completed the Modified Dental Anxiety Scale, and were interviewed privately using a reflexive, in-depth technique. All interviews were audio-tape recorded and transcribed. The transcripts were analysed to identify the impact of dental anxiety upon the participants' daily lives. Twenty-five per cent of the qualitative data was reviewed by an independent researcher to ensure the reliability of the analysis. Results The mean age of participants was 41 years (range 23 to 60). The mean MDAS score was 21.5 (range 14 to 25). Five main impacts of dental anxiety were identified: physiological; cognitive; behavioural; health; and social. Subsumed under these broad categories were: the fright response; a vast array of negative thoughts, feelings and fears; avoidance behaviour and behaviours related to eating, oral hygiene, and self-medication; and other manifestations of anxiety in the dental environment including muscular tension, crying and aggression were all identified. Dental anxiety was also found to disturb sleep and to have a profound affect socially, interfering with work and personal relationships. Conclusion The impact that dental anxiety can have on people's lives is wide-ranging and dynamic.

RESEARCH prostheticsAim To establish how widespread the emotional effects of tooth loss are. Method A questionnaire, distributed to 100 edentulous people undergoing routine prosthetic care in the Department of Prosthetic Dentistry at Guy's, King's and St Thomas' Dental Institute (GKT), was used to explore the emotional effects of tooth loss. Results Ninety four people completed the questionnaire of whom 42 stated that they had experienced difficulties in accepting the loss of their teeth. In comparison with people who had no difficulties in accepting the situation, these people were: more likely to feel less confident about themselves; more likely to feel inhibited in carrying out everyday activities; and less able to accept the inevitable change in facial shape which occurs following the loss of teeth. Additionally, they took longer to come to terms with their tooth loss (All these differences were statistically significant). Just over three-quarters of the people who were unprepared for the loss of their teeth, felt that an explanation from the dentist prior to dental extractions would have helped. Conclusion The impact that tooth loss can have on people and their lives should not be underestimated. In this study it affected 45% of the participants.I t is well established that people with dentofacial abnormalities experience social consequences including greater degrees of social avoidance and being perceived as possessing negative personality characteristics. 1 Even minor facial abnormalities can result in social stigma. 2 Visible disfigurements are recognised as having a profound effect on individuals 3 and, according to French, hidden impairments (such as total tooth loss) can cause an equal amount of stress. 4 Indeed, people with facial disfigurement are among those people who are covered by the 1995 Disability Discrimination Act. 5 While Bergendal acknowledged total tooth loss as a serious life event, 6 and the impact of dental ill health on daily living is well recognised, 7-10 it is only recently that attention has been paid to the emotional effects of tooth loss. A qualitative study into the emotional effects of total tooth loss revealed a wide range of reactions. 11 The main themes identified in relation to tooth loss were: bereavement; loss of self confidence; concerns about appearance and self image; tooth loss as a taboo subject which could not be discussed with other people; keeping tooth loss secret; the need for prosthodontic privacy; behaviour change; premature ageing; and a lack of preparation for the loss of teeth. A further qualitative study among people with partial tooth loss uncovered a similar pattern of emotional reactions. ResumenEl objetivo de este estudio fue analizar la autopercepción de la estética de la sonrisa en pacientes que acudieron a la atención odontológica de la Universidad Católica de Cuenca. Fue un estudio descriptivo de corte transversal actual. Se aplicó una encuesta a 250 pacientes de 18 a 70 años de ambos sexos que asistieron a la consulta odontológica de la clínica docente de la Universidad Católica de Cuenca en el periodo Marzo -Julio del 2017, misma que consto de 11 preguntas relacionadas a la estética dental. Se obtuvo como resultado que la autopercepción de la estética dental fue mayor para el sexo femenino en un 12 % más al comparar con el sexo masculino, en donde los factores de mayor afección fueron el color de dientes, posición en la que se encuentran, tamaño y forma en un 41,29 %, 29,03 % y 22,58 % respectivamente, los de menor afección fueron diastemas y edentulismo; las piezas dentales más relacionadas a la autopercepción de la estética de la sonrisa fueron los incisivos centrales superiores e inferiores. Así podemos decir que todos los pacientes examinados auto percibieron al menos un factor que afectó la estética de la sonrisa.Palabras clave: Estética, Autopercepción, Desdentamiento. AbstractThe aim of this study was to analyze the self-perception of smile aesthetics in patients who attended dental care at the Catholic University of Cuenca. It was a descriptive study of current cross section, a survey was applied to 250 patients from 18 to 70 years of both sexes who attended the dental clinic of the teaching clinic of the Catholic University of Cuenca in the period March -July 2017, same which consists of 11 questions related to dental aesthetics. The result was that the self-perception of dental aesthetics was greater for the female sex by 12% more when compared to the male sex, where the factors of greatest affection were tooth color, position in which they are, size and formed in 41.29%, 29.03% and 22.58% respectively, those with the least affection were diastema and edentulism; The teeth most related to the self-perception of the aesthetics of the smile were the upper and lower central incisors. So, we can say that all the patients examined self-perceived at least one factor that affected their smile aesthetics.Key words: Aesthetics, Self-perception, Edentulous.
