SummaryIn a series of 645 patients with epilepsy only 38 had attacks exclusively during sleep. A further 111 had attacks while waking and sleeping. The total number of patients whose epilepsy started with sleep attacks and who later developed attacks when waking increased with each successive year. Es wird eine Anwendung spektralanalytischer Analysemethoden für die Untersuchung von EEG-Mustern gezeigt. Gegenstand der Untersuchung sind eine 85 Sekunden dauernde, extracraniale Stimulation mit Rechteckimpulsen (Folgefrequenz 100 Hz) am Menschen, die stimulationskorrelierten Änderungen in der spontanen Kirnaktivität und der Vergleich derselben mit EEG-Mustern aus einer somnolenten Versuchsperiode. Es kann gezeigt werden, daß der elektrische Strom (Rechteckimpuls l ms, Folgefrequenz 100 Hz, l mA) zwei konträre Wirkungen auf die spontane bioelektrische Hirnaktivität ausübt, nämlich eine gleich große Dämpfung des a-und -3-Rhythmus -die wahrscheinlich durch das Blockieren des Vorverstärkers zustande kommt und daher als "Artefakt" angesehen werden kann -und eine mit einer Latenz von 40 Sekunden nach Stimulationsbeginn erscheinende Aktivierung des -3-Rhythmus, die die Stimulation um denselben Betrag (40 sec) noch überdauert. Diese Vergrößerung der $-Aktivität kann wahrscheinlich «als echte stimulationskorrelierte Beeinflussung der neuronalen Aktivität gedeutet werden.An application of spectral analysis methods to the study of EEG samples is shown. Using this method, a stimulus consisting of rectangular pulses (repetition rate 100 Hz) is applied to an intakt human skull for 85 sec. The stimulus-related changes in the spontaneous brain activity are recorded, and this recording is compared with EEG samples from recordings made during fatigue. It can be shown that electrical current (rectangular pulses 1ms, repetition rate 100 Hz, 1 mA) has two contrary effects on spontaneous brain activity. One is an equal damping of the alpha and theta rhythms, which probably occurs as a result of the blocking of the preamplifier and thus appears as an "artefact". The other is an activation of the theta rhythm with a latency of 40 sec after the beginning of the stimulus, which also outlasts the stimulus. This increase in theta activity can probably be interpreted as a real stimulus-correlated effect upon neuronal activity. Die heute allgemein unter dem Namen "Spektralanalyse" bekannte mathematische Methode igeht auf N. Wiener zurück, der 1930 das erste Mal darüber berichtete. Von Wiener stammt auch die Idee, das EEG als stationäre Zeit-Serie .(stationärer stochastischer Prozeß) zu betrachten und spektralanalytische Methoden anzuwenden. Die Spektralanalyse benötigt im ersten Schritt die Kalkulation der Autospektralfunktion (Brazier und Casby, 1952), von der mit Hilfe der «Fouriertransformierten die spektrale Leistungsdichteverteilung -gewonnen wird (Blackman und Tukey, 1958). Über die Anwendung der Spektralanalyse '«auf das EEG berichteten unter anderem Sato et al. .(I960), Adey et al. (1961), Walter (1963) und Walter und Adey {1963). Das EEG ist im wesentlichen .ein Abbild der integrativen bioelektrischen Hirntätigkeit und setzt sich als solches aus den verschieden gewichteten Anteilen einzelner mehr oder weniger weit vom Ableitort entfernter Potentialquellen zusammen. Objective The vertex sharp transient (VST) is an electroencephalographic (EEG) discharge that is an early marker of non-REM sleep. It has been recognized since the beginning of sleep physiology research, but its source and function remain mostly unexplained. We investigated VST generation using functional MRI (fMRI). Methods Simultaneous EEG and fMRI were recorded from 7 individuals in drowsiness and light sleep. VST occurrences on EEG were modeled with fMRI using an impulse function convolved with a hemodynamic response function to identify cerebral regions correlating to the VSTs. A resulting statistical image was thresholded at Z>2.3. Results Two hundred VSTs were identified. Significantly increased signal was present bilaterally in medial central, lateral precentral, posterior superior temporal, and medial occipital cortex. No regions of decreased signal were present. Conclusion The regions are consistent with electrophysiologic evidence from animal models and functional imaging of human sleep, but the results are specific to VSTs. The regions principally encompass the primary sensorimotor cortical regions for vision, hearing, and touch. Significance The results depict a network comprising the presumed VST generator and its associated regions. The associated regions functional similarity for primary sensation suggests a role for VSTs in sensory experience during sleep.
