“…The clinical expertise in HNC within OncoAge is highlighted by the ELAN (ELderly Head and Neck cancer) program. These clinical studies on curative or palliative personalized treatment of elderly head and neck cancer patients after geriatric assessment (Elan Geriatric Evaluation, EGE) currently represent the only multi-center therapeutic trials dedicated to this group of patients worldwide [34,35,36]. Studies were completed in 2018 and results will be presented in 2019.…”

“…That is why a CGA by an onco-geriatrician should be part of the HNC MDT (116). Recently, the ELAN-ONCOVAL (Elderly head and Neck Cancer-Oncology eValuation) study evaluating the use of a suited geriatric evaluation to stratify patients and guide therapy in patients ≤70 years old not amenable to surgery showed that, despite the initially planned treatment was changed only in 8% of the cases after a geriatric assessment, the number of patients requiring multidisciplinary interventions was significantly higher when the assessment was performed by geriatrician (71 vs. 51%) (117). The results of this study highlight the relevance of incorporating the onco-geriatrician in the decision making process for the elderly.…”

“…Non-oncologic GA-guided interventions are also common and feasible in daily practice: these include physical therapy referrals, nutritional care, psychiatric and neuropsychological consultations, social support assistance [9,10]. The role of CGA remains controversial, and there is still no validation in the HNSCC setting: preliminary results of a study with CGA in HNSCC patients showed that CGA changed treatment decisions in 8% of patients [11,12], but as of now, there is no evidence it improves results in HNSCC patients. Nevertheless, according to many experts, all patients should undertake the CGA before treatment.…”

“…G8 includes only eight items: age, decreased oral intake, weight loss in the past 3 months, mobility, neuropsychological problems, BMI, ≥3 medications daily, and self-reported overall health. Nevertheless, the ELAN-ONCOVAL trial, published in 2019, endorsed the use of the EGE (ELAN geriatric evaluation) in lieu of the G8 screening tool, which was considered not appropriate for elderly HNSCC patients [11,12]. In the locally advanced (LA) HNSCC setting, small studies using GA have been conducted.…”
