The EKG Method for Positioning the Tips of PICCs: Results from Two Preliminary Studies

Abstract: Two preliminary studies were conducted to determine feasibility of using the electrocardiography (EKG) method to determine terminal tip location when inserting a peripherally inserted central catheter (PICC). This method uses the guidewire inside the catheter (or a column of saline contained in the catheter) as an intracavitary electrode. The EKG monitor is then connected to the intracavitary electrode. The reading on the EKG monitor reflects the closeness of the intracavitary electrode (the catheter tip) to t… Show more

“…Between January 1, 2007, and May 31,2009, 225 PICCs were placed by using this method in our department. Of these, 33 tips (14.7%) were initially malpositioned (not in the lower third of the SVC).…”

“…Still other devices can help locate the tip of a PICC, such as the Sherlock Tip Location System 42 and the Electromagnetic Detection System, 41 but these private devices are expensive and the operator needs special training. 31,42,43 On the other hand, radiographic confirmation of the tip location cannot be omitted even if the PICC was inserted with the help of all these devices. According to the Infusion Nurses Society's 2006 standards of practice, 19 locations of all central catheter tips shall be determined radiographically and documented before the start of the prescribed therapy.…”

“…), both with open-ended or closed ended tip. The electrocardiographic methodology presents several important features: has the same accuracy of the fluoroscopy, however, it is easier to use, more promptly accessible, more secure and, therefore, has a major cost-effectiveness [63,[71][72][73][74][75][76] (Figure 7). …”
