The pilot study assessed the utility of a training program using modified, commercially available dual-joystick-operated ride-on toys to promote unimanual and bimanual upper extremity (UE) function in children with cerebral palsy (CP). The ride-on-toy training was integrated within a 3-week, intensive, task-oriented training camp for children with CP. Eleven children with hemiplegia between 4 and 10 years received the ride-on-toy training program 20–30 min/day, 5 days/week for 3 weeks. Unimanual motor function was assessed using the Quality of Upper Extremity Skills Test (QUEST) before and after the camp. During ride-on-toy training sessions, children wore activity monitors on both wrists to assess the duration and intensity of bimanual UE activity. Video data from early and late sessions were coded for bimanual UE use, independent navigation, and movement bouts. Children improved their total and subscale QUEST scores from pretest to post-test while increasing moderate activity in their affected UE from early to late sessions, demonstrating more equal use of both UEs across sessions. There were no significant changes in the rates of movement bouts from early to late sessions. We can conclude that joystick-operated ride-on toys function as child-friendly, intrinsically rewarding tools that can complement conventional therapy and promote bimanual motor functions in children with CP.

Bu çalışmanın amacı adölesan Serebral Palsi (SP)’li bireylerde fonksiyonel motor ve iletişim performansı etkileniminin Sağlıkla İlişkili Yaşam Kalitesi (SİYK) sonuçlarına etkisini araştırmaktır.Çalışmaya yaş aralığı 13-18 (ortalama [O] 16,45 ve standart sapma [SS]: 1,8; 24 erkek ve 27 kız) arasında değişen ve büyük çoğunluğu spastik SP olmak üzere (%68,6), SP’nin diğer alt türleri (diskinetik: %23,5; ataksik: %7,8) ile tanılı adölesan SP’li bireyler dahil edildi. Bireylerin fonksiyonel etkilenim şiddeti El Becerileri Sınıflandırma Sistemi (EBSS), Kaba Motor Fonksiyon Sınıflandırma Sistemi (KMFSS) ve İletişim Becerileri Sınıflandırma Sistemi (İBSS) kullanılarak belirlendi. SİYK sonuçları ise Adölesanlar için Serebral Palsi Yasam Kalite Anketi (CP QOL-Ergen) sürümü kullanılarak tanımlandı. Yüksek ve düşük fonksiyonel seviyelerdeki bireylerin SİYK sonuçları arasındaki olası istatistiksel farklar Mann Whitney-U testi kullanılarak araştırıldı. Kaba ve ince motor becerilerindeki etkilenim şiddetinin SİYK’in tüm alt parametrelerine anlamlı etkisi bulundu (P<0,05). Benzer şekilde, CP QOL- ‘Okul Durumu’ ve ‘İşlevsellik Hakkındaki Düşünceler’ alt testleri (P>0,05) dışındaki diğer tüm SİYK alt testlerinde iletişim becerilerindeki etkilenim şiddeti bakımından gruplar arasında istatistiksel olarak anlamlı farklılıklar saptandı (P<0,05).Adölesan SP’li bireylerde daha iyi SİYK sonuçları daha iyi bir fonksiyonel motor ve iletişim performansı seviyesine bağlıdır. Bundan ötürü, SP’li ergenlerin hem fonksiyonel hem de iletişim becerilerini artıracak en uygun tedavi yaklaşımları ile desteklenmeleri daha iyi SİYK sonuçlarına sahip olmaları açısından çok önemlidir.

Validity and Reliability of the Turkish Version of the KIDSCREEN-27 for Individuals With Cerebral Palsy

Our aims in this study were to examine the construct/concurrent validity and internal/test-re-test reliabilities of both the self-report and parent-report questionnaires of a Turkish version of the KIDSCREEN-27 for adolescents with cerebral palsy (CP). We used a convenience sample of 135 children and adolescents with CP aged 8–18 years (65 males, 70 females; M age = 12.39, SD = 3.57) and their parents/caregivers (123 mothers, seven fathers, and five grandmothers). We explored structural construct validity via confirmatory factor analysis (CFA). Concurrent validity was examined via Spearman’s correlations between the KIDSCREEN-27 questionnaires and the self-report and primary caregiver report forms of the Cerebral Palsy Quality of Life for Children (CP QOL-Child) and adolescents (CP QOL-Teen). We explored test-retest and internal consistency reliabilities utilizing intraclass correlation coefficients (ICC) and Cronbach’s alpha (α), respectively. CFA goodness-of fit indices verified that the predefined model of the KIDSCREEN-27 was a good fit for data from the CP population (X2/df<5, GFI >0.90, AGFI >0.90, RMSEA<0.80). Results showed the subdomains of both KIDSCREEN-27 forms to be significantly correlated with the matched subdomains of the CP QOL-Child and CP QOL-Teen questionnaires (ranges of r = 0.62–0.94). Overall, α and ICC coefficients for all subdomains of both KIDSCREEN-27 versions were acceptable (α and ICC >0.70). Both versions of the KIDSCREEN-27 have satisfactory psychometric properties for use in evaluating health-related quality of life (HRQOL) outcomes in children and adolescents with CP.
