The efficacy of the Cook-Swartz implantable Doppler in the detection of free-flap compromise: a systematic review protocol

Abstract: IntroductionThe Cook-Swartz implantable Doppler monitors venous or arterial blood flow from free flaps and can detect free-flap compromise. Previous studies have shown that the use of this Doppler can improve detection and salvage rates as it provides an earlier warning than the current method of clinical assessment. Such studies assert that the implantable Doppler is of great value in monitoring free flaps in current microsurgical units. This systematic review aims to compare the efficacy of the Cook-Swartz i… Show more

“…Moreover, as in physical examination, this method is burdensome to providers, who may be performing serial monitoring while caring for other patients. In the last 10 years, implantable Doppler has gained popularity, as this method allows for continuous monitoring and improvement in salvage rates and has been particularly useful for monitoring buried flaps . However, implantable Doppler devices may dislodge, malfunction, or injure an anastomosis upon removal .…”

“…The major limitation of non‐invasive techniques is their inability to monitor flaps that are buried, which require invasive approaches for assessment. The implantable Doppler system is the most commonly used invasive monitor and is reliable across a wide range of reconstructive procedures . Specifically, it is associated with increased flap salvage rates and low complication rates .…”

“…The implantable Doppler system is the most commonly used invasive monitor and is reliable across a wide range of reconstructive procedures. 7,9,[17][18][19] Specifically, it is associated with Left ankle 1 (1) increased flap salvage rates and low complication rates. 20 An analysis that examined its efficacy in 547 consecutive, non-buried free flaps concluded that there is a strong trend toward improved salvage rates with implantable Doppler monitoring compared to clinical monitoring (80% vs 66%, P = 0.48) without increasing the rate of false-positive operative re-explorations.…”
