In recent years, there has been an increase in the prevalence of mental disorders connected with affective dysregulation and insecure attachment. Therefore, it is even more important to understand the interplay between an individual’s attachment representation and patterns of emotion regulation. To our knowledge, this is the first systematic review to examine this association. PsycInfo, PsyArticles, and PubMed were searched for studies that examined attachment-related differences in emotion regulation in adults. To examine the unconscious attachment representation, only studies using the Adult Attachment Interview or the Adult Attachment Projective Picture System were included. Thirty-seven peer-reviewed studies (with a total of 2006 subjects) matched the PICO criteria. Emotion regulation was measured via four objective approaches: autonomic nervous system, brain activity, biochemistry, or nonverbal behavior. Across all measurements, results reveal a significant correlation between attachment representation and emotion regulation. Secure attachment correlates consistently with balanced emotion regulation, whereas it is impaired in insecure and dysfunctional in unresolved attachment. Specifically, unresolved individuals display counterintuitive responses and fail to use attachment as a resource. Insecure-dismissing attachment is associated with an emotionally deactivating strategy, while on a physiological, biochemical, and nonverbal level, emotional stress is still present. There is still a lack of studies examining preoccupied individuals. In addition to interpreting the results, we also discuss the risk of bias, implications for psychotherapy and coaching, and an outlook for future research.

Importance: In 2021, the American Academy of Pediatrics published a policy statement seeking to create a paradigm shift away from a focus on childhood toxic stress and toward the emphasis on early relational health (ERH) as a buffer for childhood adversity and promoter of life-course resilience. A comprehensive appraisal of the efficacy of contemporary parent/caregiver-child interventions in — primarily — improving ERH, and — secondarily — enhancing child wellbeing and neurodevelopment is needed to guide widespread implementation and policy. Objective: Determine the effectiveness of contemporary early dyadic parent/caregiver-infant interventions on ERH, child socio-emotional functioning and development, and parent/caregiver mental health. Data Sources: PubMed, Medline, Cinhal, ERIC, and PsycInfo were searched on April 28, 2022. Additional sources: clinical trial registries (clinicaltrials.gov, ISRCTN Registry, EU Clinical Trials Register, Australian New Zealand Clinical Trials Registry), contacting authors of unpublished/ongoing studies, backward/forward reference-searching. Study Selection: Studies targeting parent/caregiver-infant dyads and evaluating effectiveness of a dyadic intervention were eligible. Study selection was performed in duplicate, using Covidence. Data Extraction and Synthesis: Cochrane methodological guidance presented per PRISMA guidelines. Data extraction and risk of bias assessment were completed in duplicate with consensuses by first author. Data were pooled using inverse-variance random effects models. Main Outcomes and Measures: The primary outcome domain was ERH. Secondary outcome domains were child socio-emotional functioning and development, and parent/caregiver mental health, and were only considered in studies where at least one ERH outcome was also measured. The association between dose of intervention and effect estimates was explored. Results: 93 studies (14,993 parent/caregiver-infant dyads) met inclusion criteria. Based on very low to moderate quality of evidence, we found significant non-dose-dependent intervention effects on several measures of ERH, including bonding, parent/caregiver sensitivity, attachment, and dyadic interactions, and a significant effect on parent/caregiver anxiety, but no significant effects on other child outcomes. Conclusion: Current evidence does not support the notion that promoting ERH through early dyadic interventions ensures optimal child development, despite effectively promoting ERH outcomes. Given the lack of an association with dose of intervention, the field is ripe for novel, innovative, cost-effective, potent ERH intervention strategies that effectively and equitably improve meaningful long-term child outcomes.

Zusammenfassung Ziele Frauen mit postpartalen psychischen Störungen zeigen häufig eine verzögerte Bindungsentwicklung zum Kind mit negativen Folgen für die kindliche Entwicklung. In mehreren Ländern wurde nachgewiesen, dass eine spezifische Mutter-Kind-Behandlung positiv auf die mütterliche Psychopathologie und die Bindungsentwicklung wirkt. Daten für den deutschsprachigen Raum sind rar, auch aufgrund der fehlenden Angebote bei unzureichender Finanzierung. Patientinnen einer psychosomatisch-psychotherapeutischen Mutter-Kind-Tagesklinik werden mit dieser Studie charakterisiert und die Behandlung evaluiert. Methodik 270 Patientinnen wurden bei Aufnahme und Entlassung aus der Tagesklinik befragt. Die Begleitevaluation umfasste die klinischen Haupt- und Nebendiagnosen nach ICD-10, Angaben zur Behandlungsdauer, Medikation, Angaben zum Kind sowie psychometrische Fragebögen zu mütterlicher Psychopathologie sowie zur wahrgenommenen Bindung zum Kind und dem elterlichen Kompetenzerleben. Ergebnisse 75% der behandelten Frauen wiesen mehr als eine, im Mittel 2,3 psychische Diagnosen auf. Die häufigsten Hauptdiagnosen waren affektive Störungen (38,5%), neurotische, Belastungs-und somatoforme Störungen (30,7%) sowie Persönlichkeits- und Verhaltensstörungen (20,4%). Ca. 56% berichteten Störungen der Mutter-Kind-Bindung. Die durchschnittliche Therapiedauer betrug 32 Behandlungstage. Zwischen Aufnahme und Entlassung zeigte sich eine hochsignifikante Symptomverbesserung mit sehr hoher Effektstärke [F=288,557 (df=1), p<0,001, Eta²=0,549]. Zur Entlassung wiesen 86,6% unserer Patientinnen keine Mutter-Kind-Bindungsstörung mehr auf. Diskussion Die Ergebnisse weisen auf potentiell hohe therapeutische Effekte der bindungsfokussierten und interaktionszentrierten Behandlung für die seelische Gesundheit der Mutter, ebenso wie die für die Bindungsentwicklung zum Kind hin. Schlussfolgerung Die gemeinsame Behandlung von Mutter und Kind sollte ein fester und finanzierter Bestandteil des Versorgungssystems sein, um Chronifizierung und negative Entwicklungsfolgen für das Kind zu verhindern.
