The efficacy of open partial aponeurectomy for recurrent Dupuytren’s contracture

Abstract: Open partial aponeurectomy performed by a board certified hand surgeon proved to be safe. The postoperative functional outcome seemed to be related to the individual course of the disease.

“…However, neither in patients with an improved nor in patients with a worsened DASH-score grip strength was significantly changed after limited aponeurectomy. This notion is well in line with the recently reported loss of grip strength as assessed by the Jamar dynamometer and pinch strength relative to the contralateral hand in patients with recurrent contractures (32). Thus, flexion contracture and impaired gripping rather than grip strength might be the main problem of patients with Dupuytren's contracture.…”

“…Dupuytren's disease is a benign connective tissue disorder affecting the palmar fascia [1][2][3][4] that can lead to flexion contracture of the metacarpophalangeal (MP) joints and the proximal interphalangeal (PIP) joints [5][6][7][8][9][10][11][12]. Controversy exists about the relative efficacy of injectable collagenase Clostridium histolyticum (CCH) and limited aponeurectomy in the treatment of Dupuytren's contracture in long term [13][14][15][16]. Recently, Smeraglia et al performed a comprehensive review of studies dealing with CCH for Dupuytren's disease [17].…”

“…15,17 Qualitatively, the rate of satisfaction ranged from 56% to 100%. 18,[20][21][22] A mean QuickDASH score of 16.5 was reported in 1 study. 15 Other studies reported DASH scores where a score of 0 shows no disability and a score of 100 complete disability, as median 17 (33) or mean 18,20,22 (range from 15 to <35.04).…”
