The Efficacy of MRI in the diagnostic workup of cystic fibrosis-associated liver disease: A clinical observational cohort study

Abstract: • Using a multivariate classification analysis, we identified three independent imaging features, altered gallbladder morphology (GBAM), periportal tracking (PPT) and periportal fat deposition (PPFD), that could diagnose CFLD with high sensitivity, 94.1 % (95% CI: 71.3-99.9) and moderate specificity, 84.6 % (95% CI: 54.6-98.1). • Based upon the results of this study, gadoxetic acid-enhanced MRI with DWI is able to diagnose early-stage CFLD, as well as its progression.

“…With multiparametric MRI, including T1 and T2, chemical shift imaging (CSI) as well as diffusion weighted imaging (DWI) and dynamic contrast-enhanced imaging, the anatomy and pathology of the liver and the biliary tree can be accurately assessed. [5][6][7] Furthermore, because of its high soft tissue contrast, MRI can even be performed without administration of contrast media in those patients with contraindications. Several methods have been described that quantify sarcopenia on cross-sectional imaging with the skeletal muscle index (SMI) being the most commonly used one.…”

“…However, MRI is increasingly used in the diagnostic workup of patients with CLD. With multiparametric MRI, including T1 and T2, chemical shift imaging (CSI) as well as diffusion weighted imaging (DWI) and dynamic contrast‐enhanced imaging, the anatomy and pathology of the liver and the biliary tree can be accurately assessed 5–7 . Furthermore, because of its high soft tissue contrast, MRI can even be performed without administration of contrast media in those patients with contraindications.…”

“…There is, however, not a single diagnostic test to screen all patients. Poetter‐Lang et al demonstrated 3 characteristics seen on magnetic resonance imaging (MRI) in CFLD, including the presence of altered gallbladder morphology, periportal tracking (a bandlike hyperintense signal alteration along the portal triad), and periportal fat deposition. Although these imaging features alone are not specific to CFLD and can be seen in any of the cholestatic liver diseases, in a patient with CF, they can be important markers for early detection for CFLD (Fig.…”

“…Prospective assessment of the algorithm demonstrated a sensitivity and specificity of 94.1% and 84.6%, respectively, in the diagnosis of CFLD. (24) The variability in liver disease manifestation in CF contributes to the challenges in establishing a definition of CF-related liver disease. As a result, the criteria to define CFLD have not been clearly established.…”

“…In this constellation imaging methods without radiation exposure should be chosen. Beside conventional ultrasonography also additional sonography methods like point shear wave elastography (pSWE) and MRI imaging, especially MR cholangiopancreatography (MRCP) may be eligible for this group of patients, both without radiation exposure [ 6 , 7 ].…”
