Цель. Оценка антиаритмической эффективности хирургической радиочастот-ной аблации (РЧА) устьев легочных вен, выполняемой в ходе аортокоронар-ного шунтирования (АКШ), а также их влияния на структурно-функциональные и электрофизиологические показатели сердца у больных с пароксизмальной формой фибрилляции предсердий (ФП). Материал и методы. В открытое клиническое исследование было включено 116 больных (71 мужчина и 45 женщин) в возрасте от 43 до 65 лет (средний возраст -57,6±6,4 лет), которым в плановом порядке выполнялось АКШ в сочетании с РЧА. По данным длительного Холтеровского мониторирования ЭКГ спустя 12 месяцев после операции больных разделили на 2 группы: 1-я группа -88 больных со стабильно синусовым ритмом и 2-я группа -28 больных с рецидивами ФП. Результаты. Показано, что у больных с пароксизмальной формой ФП через 12 мес. после РЧА в сочетании с АКШ стабильный синусовый ритм сохраняется в 75,9% случаев (1-я группа). После комбинированного вмешательства в обеих группах показатели эффективного рефрактерного периода предсердий и частотный порог индуцирования аритмии уменьшились достоверно, диспер-сия зубца Р, наоборот, увеличилась, а также выявлены межгрупповые различия этих показателей. Кроме того, в 1-й группе стабилизации синусового ритма способствовало значительное улучшение показателей диастолической функ-ции левого желудочка, которые были достоверно лучше, чем во 2-й группе. Заключение. Установлено, что сочетание РЧА устьев легочных вен с опера-цией АКШ у больных с пароксизмальной ФП способствует улучшению струк-турно-функционального и электрофизиологического состояния сердца и, тем самым, сохранению стабильного синусового ритма.
