The Efficacy of Continuous Local Arterial Infusion of 5-Fluorouracil and Cisplatin through an Implanted Reservoir for Severe Advanced Hepatocellular Carcinoma

Abstract: There is not yet an effective standard therapy for severe advanced hepatocellular carcinoma (HCC). We attempted continuous local arterial infusion chemotherapy using 5-fluorouracil (5-FU) and cisplatin (CDDP) with an implanted reservoir for these patients, and evaluated its efficacy. Twenty-one HCC patients received continuous arterial infusion of 5-FU and CDDP for 1 week, followed by a 1-week no-infusion period; this regimen was repeated 1-32 times, and patients were observed for 36-549 days. The 1-year survi… Show more

“…In each of our 55 patients, an intra-arterial catheter was inserted through the subclavian or femoral artery with a subcutaneously implanted drug delivery system (Toyoda et al, 1995). Each patient was treated with subcutaneous IFN-a (OIF, Otsuka Pharmaceutical Co., Tokushima, Japan) and intra-arterial infusion of 5-FU (Kyowa Hakko Co., Tokyo).…”

“…2,3 Several nonsurgical treatments have been attempted, including transcatheter arterial chemoembolization (TACE), transcatheter arterial embolization (TAE), 4,5 percutaneous ethanol injection (PEIT), 6,7 percutaneous microwave coagulation therapy (PMCT), 8 or most recently, radiofrequency ablation (RFA). 9,10 Sys-temic or hepatic arterial infusion chemotherapy 11,12 is also attempted for patients unable to undergo other surgical or nonsurgical treatment mainly because they have advanced stage disease.…”

“…RHAIC with a subcutaneously implanted injection port was initiated at the study institution (Ogaki Municipal Hospital, Ogaki, Japan) in 1990. 11 In Japan, it currently replaces systemic chemotherapy as a treatment option for patients who are unable to undergo surgery, PEIT/PMCT/RFA, or TACE/TAE. 11,12,24 -26 Using the Kaplan-Meier comparison, patient survival rates significantly improved according to the period of the year of HCC diagnosis among the first three periods.…”

“…Concerning the multidrug regimens containing CDDP for HAI therapy, low-dose CDDP combined with 5-FU (low-dose FP) has been intensively investigated in recent years [23,24,26,27,29,30,32,33] (Table 2). A response rate of about 40% and a median survival time of 6-12 mo have been reported for this treatment.…”
