Objective: To compare the efficacy of 5% potassium hydroxide (KOH) and 10% potassium hydroxide (KOH) solution for the treatment of molluscum contagiosum at a tertiary care Hospital. Study Design: Quasi-experimental study. Place and Duration of Study: Dermatology Department, Tertiary Care Hospital Karachi, from Oct 2019 to Apr 2021. Methodology: Sixty patients with molluscum contagiosum who fulfilled the inclusion and exclusion criteria were enrolled in the outpatient department of tertiary care hospital in Karachi. Patients were divided into two groups, A (5% KOH) and B (10% KOH) of 30 patients each. Potassium Hydroxide (KOH) solution was applied at home using the cotton-tipped applicator twice daily. Follow-up of patients was done at 2-weekly intervals for 12 weeks. Efficacy was measured based on the resolution of lesions or improvement from baseline. Results: Complete response occurred in 23 (76.7%) cases in 10% KOH Group while 6 (20%) in 5% KOH Group, Partial response occurred in 6 (20%) in 10% KOH group while 15 (50%) in 5% KOH group, No response occurred in 1 (3.33%) in 10% KOH group and 9 (30%) in 5% KOH group (p<0.001). Conclusion: 10% Potassium Hydroxide (KOH) is an efficient modality for the treatment of molluscum contagiosum compared to 5% Potassium Hydroxide (KOH).

Context: Molluscum contagiosum (MC) is a common viral cutaneous infection. Despite multiple treatment options, there is no definitive treatment. In some cases, the lesions are severe, recurrent, and cosmetically odd. Modified autoinoculation (MAI) is a novel technique that induces cell-mediated immunity resulting in clearance of local as well as distant lesions. Potassium hydroxide (KOH) acts by dissolving the keratin and penetrating deeply destroys the hyperproliferative tissue. We would here like to compare MAI with topical KOH in the treatment of MC. Aims and Objective: The aim of this study was to assess the effectiveness of MAI in treatment of MC and to compare its response with topical KOH application. Settings and Design: This was an open-labeled longitudinal therapeutic outcome study carried out at a tertiary care center over a period of 1 year. Materials and Methods: Hundred consenting MC patients attending the department of dermatology were randomized into Group A and Group B. Group A patients were subjected to MAI and Group B to topical application of 10% KOH. Statistical Analysis Used: The continuous variables are presented as mean ± standard deviation (SD). The difference between the mean score was analyzed using Student’s t test for independent variable and paired t test for paired results. Results: At the end of 16 weeks, 91.48% showed complete clearance by MAI compared to 81.64% with topical 10% KOH solution. There was a significant reduction of mean score of lesions in patients treated by MAI compared to patients treated with KOH. Conclusion: MAI therapy provides a promising, easy, cost-effective, daycare option for MC infections.

Background: Molluscum Contagiosum (MC) has no FDA-approved treatments as of yet. Although many other treatments have been suggested, the majority of the efficacy evidence comes from modest case studies. Objectives: The aim of the current work was to evaluate the safety as well as efficacy of intralesional candida Ag immunotherapy versus topical KOH (10 %) in the treatment of MC. Patients and methods: A total of 40 patients (21females and 19 males) with multiple lesions of MC. They were divided into two equal groups at random; group 1 treated by candida antigen at 2 weeks intervals with a maximum of five injections or clinical cure. Group 2 subjected to topical 10% KOH twice daily for ten weeks. Results: Complete clinical response was attained in 80% of MC patients who treated with candida antigen for 10 weeks versus 85% of patients who treated with topical KOH for 10 weeks, while 20 % of patients showed no response in group 1 versus 5% in group 2 after the end of therapy. without recurrence after 3 months in all patients demonstrating complete reaction. In group 1, most patients tolerated the side effects, which in 5% of cases manifested as erythema and hyperpigmentation. While in group 2, hypopigmentation (15%), hyperpigmentation (10%), pruritus (10%) during treatment were reported. There was significant reported burning sensation among 25% of patients treated with topical potassium hydroxide (P = 0.047). Conclusion:It could be concluded that both topically applied KOH10% and intralesional candida are dependable, less expensive treatments for non-genital MC.
