Efficacy of acupuncture for lifestyle risk factors for stroke: A systematic review

BackgroundModifications to lifestyle risk factors for stroke may help prevent stroke events. This systematic review aimed to identify and summarise the evidence of acupuncture interventions for those people with lifestyle risk factors for stroke, including alcohol-dependence, smoking-dependence, hypertension, and obesity.MethodsMEDLINE, CINAHL/EBSCO, SCOPUS, and Cochrane Database were searched from January 1996 to December 2016. Only randomised controlled trials (RCTs) with empirical research findings were included. PRISMA guidelines were followed and risk of bias was assessed via the Cochrane Collaboration risk of bias assessment tool. The systematic review reported in this paper has been registered on the PROSPERO (#CRD42017060490).ResultsA total of 59 RCTs (5,650 participants) examining the use of acupuncture in treating lifestyle risk factors for stroke met the inclusion criteria. The seven RCTs focusing on alcohol-dependence showed substantial heterogeneity regarding intervention details. No evidence from meta-analysis has been found regarding post-intervention or long-term effect on blood pressure control for acupuncture compared to sham intervention. Relative to sham acupuncture, individuals receiving auricular acupressure for smoking-dependence reported lower numbers of consumed cigarettes per day (two RCTs, mean difference (MD) = -2.75 cigarettes/day; 95% confidence interval (CI) = -5.33, -0.17; p = 0.04). Compared to sham acupuncture those receiving acupuncture for obesity reported lower waist circumference (five RCTs, MD = -2.79 cm; 95% CI: -4.13, -1.46; p<0.001). Overall, only few trials were considered of low risk of bias for smoking-dependence and obesity, and as such none of the significant effects in favour of acupuncture interventions were robust against potential selection, performance, and detection bias.ConclusionsThis review found no convincing evidence for effects of acupuncture interventions for improving lifestyle risk factors for stroke.

Resumo: Contextualização: O desejo de melhoras nos aspectos estéticos tem sido foco feminino. Na mulher, o tecido adiposo se concentra com maior intensidade nos quadris, glúteos e abdômen. A acupuntura tem se mostrado uma possibilidade de recurso complementar em estética. Objetivo: Avaliar os resultados da eletroacupuntura na redução de medidas abdominais. Metodologia: Ensaio clínico experimental, com grupo controle. Amostra com 20 mulheres com IMC entre 19,4 a 27,7 e idades de 23 a 36 anos, distribuídas por sorteio em 2 grupos iguais, grupo teste e grupo controle. Utilizaram-se agulhas descartáveis 0,30×30mm e 0,30×50 mm da marca Dong Bang R e eletroestimulador para acupuntura da marca NKL R , modelo Ominipax. Utilizou-se a mensuração da circunferência abdominal, sendo feita três medidas; 5 cm acima da cicatriz umbilical, na cicatriz umbilical e 5 cm abaixo da cicatriz umbilical. Realizaram-se 10 sessões, 2× por semana, sendo que foi estimulados a eletroacupuntura em pontos locais do abdômen, com estímulo contínuo 300Hz por 20 min. e agulhamento sem estímulo em IG4, E36, BP6, C7, VG20 e Yintang. O tempo de retenção das agulhas foi 20 min. Resultados: O grupo teste teve 50% dos voluntários com redução de 5 a 7% de medidas abdominais e os outros 50% de 1 a 4%. No grupo controle, 70% das voluntárias não obtiveram nem ganho e nem perda de medidas abdominais, 20% obtiveram 1% de perda e 10% teve aumento de medidas abdominais. Conclusão: Embora discretos os percentuais de redução de medidas, eles foram superiores no grupo que recebeu eletroacupuntura.

Objectives: Complementary medical methods such as auricular acupuncture are used for the treatment of obesity in addition to classical methods. Our aim was to compare the effects of exercise plus diet applications with exercise plus diet plus auricular acupuncture applications in the obese patients. Material and Methods: Seventy volunteer women aged between 20-60 years with BMI ≥ 30 participated in the study. Volunteers were randomly assigned into two groups as Diet-Exercise (DE) and Acupuncture-Diet-Exercise (ADE). Body weight, body mass index, body fat mass and basal metabolic rate measurements were recorded at the beginning of the study and during five months in monthly periods. Both groups were given a dietary program equivalent to basal metabolic rate and exercise of 200 kcal/day for five days per week. In addition, auricular acupuncture was administered to the ADE group. Results:Statistically significant weight loss was observed in the monthly periods in both groups (p<0.05), but there was no statistically significant difference between the two groups. Conclusion: According to these results, weight loss can be achieved by using diet and exercise together, while four point applied permanent auricular acupuncture does not seem to make any extra contribution to weight lost.
