Tuberous sclerosis complex (TSC) is an autosomal dominant neurocutaneous disease affecting approximately 1 in 6,000 people, and represents one of the most common genetic causes of epilepsy. Epilepsy affects 90% of the patients and appears in the first 2 years of life in the majority of them. Early onset of epilepsy in the first 12 months of life is associated with high risk of cognitive decline and neuropsychiatric problems including autism. Prenatal or early infantile diagnosis of TSC, before the onset of epilepsy, provides a unique opportunity to monitor EEG before the onset of clinical seizures, thus enabling early intervention in the process of epileptogenesis. In this review, we discuss the current status of knowledge on epileptogenesis in TSC, and present recommendations of American and European experts in the field of epilepsy.

ÖzetYüksek yağ ve düşük karbonhidrat içeren ketojenik diyet 1920'lerin başında epilepsi tedavisinde kullanılmaya başlanmıştır. Fenitoin gibi yeni ilaçların keşfiyle 90'lara kadar unutulmuş eski bir tedavi olarak kalmıştır. Dirençli epilepside ketojenik diyet ile hastaların yarısının nöbet sayılarında belirgin azalma ve yaklaşık %15'inde ise tam nöbet kontrolü bildirilmiştir. Önceleri son tedavi tercihleri arasında yer alan diyet, artan bilgi birikimi ve daha kolay uygulanabilir değiştirilmiş formların da etkinliğinin görülmesiyle miyoklonik, astatik epilepsi, Dravet sendromu ve süt çocuğu spazmı gibi epileptik ansafalopatilerde ilk tedavi seçenekleri arasına yerleşmiştir. As new antiepileptic agents such as phenytoin were discovered, popularity of the diet was waned until 90's. Studies on efficacy of diet in refractory epilepsy reported that approximately half of the children had a significant reduction, 15% of whom were seizure free. With growing body of evidence and introduction of diet modifications, ketogenic diet was promoted as a first line treatment for epileptic encephalopathies such as myoclonic-astatic epilepsy , Dravet syndrome and infantile spasm. Purpose: To provide primary care providers with a general overview of the genetic disorder tuberous sclerosis complex (TSC). By understanding the disease mechanism for this genetic condition, providers can effectively care for TSC patients and properly educate families who are affected by TSC. The article also describes the multi-system clinical presentation of the disease to assist primary care providers with an early diagnosis. Data Resources: Research articles and evidence-based guidelines found through MEDLINE and the World Wide Web. Conclusions: Using various diagnostic tools and treatment options, providers can offer the multidisciplinary approach needed to manage this disease appropriately. New treatment options, such as rapamycin, may be the future drug of choice in treating TSC. Implications for practice: By following evidence-based clinical practice guidelines, providers can hope to reduce TSC-related morbidity and mortality.
