Assessing the effect of empathy-enhancing interventions in health education and training: a systematic review of randomised controlled trials

ObjectiveTo estimate the effect of empathy interventions in health education and training from randomised controlled trials (RCTs).MethodsMEDLINE, PsycINFO, EMBASE, CINAHL and Cochrane databases were searched from inception to June 2019 for RCTs investigating the effect of empathy-enhancing interventions in medical and healthcare students and professionals. Studies measuring any aspect of ‘clinical empathy’ as a primary or secondary outcome were included. Two reviewers extracted data and assessed the risk of bias of eligible studies using the Cochrane Risk of Bias Tool. Random effects meta-analyses of the impact of empathy training on participants’ empathy levels were performed.ResultsTwenty-six trials were included, with 22 providing adequate data for meta-analysis. An overall moderate effect on participant empathy postintervention (standardised mean difference 0.52, 95% CI 0.36 to 0.67) was found. Heterogeneity across trial results was substantial (I2=63%). Data on sustainability of effect was provided by 11 trials and found a moderate effect size for improved empathy up until 12 weeks (0.69, 95% CI 0.23 to 1.15), and a small but statistically significant effect size for sustainability at 12 weeks and beyond (standardised mean difference 0.34, 95% CI 0.11 to 0.57). In total, 15 studies were considered to be either unclear or high risk of bias. The quality of evidence of included studies was low.ConclusionFindings suggest that empathy-enhancing interventions can be effective at cultivating and sustaining empathy with intervention specifics contributing to effectiveness. This review focuses on an important, growing area of medical education and provides guidance to those looking to develop effective interventions to enhance empathy in the healthcare setting. Further high-quality trials are needed that include patient-led outcome assessments and further evaluate the long-term sustainability of empathy training.Protocol registration numberPROSPERO (CRD42019126843).

Background. According to the biopsychosocial approach and patient-centred models, efficient management of relational competences by healthcare professionals is crucial to affect a patient’s treatment and care process positively. Empathy is one of the major components of the relational skills expected of health professionals. The knowledge of the empathic ability of undergraduate healthcare students represents important information for educators in defining specific and efficient educational programs aimed at supporting or enhancing empathic competences. In this study, we measured the attitude towards the value of health professional empathy in clinical encounters of the first-year undergraduate nursing students. Motivations for a nursing education choice were also evaluated. We expected a significant association between students’ positive attitude towards the value of health professional empathy and the prosocial and altruistic motivations in choosing nursing studies. Methods. A cross-sectional study was performed on 77 first-year nursing students. The Jefferson Scale of Empathy – Health Professions Student Version was administered. The motivations for choosing nursing studies were detected through an open question.Results. Seven distinct themes, distinguishing between internal and external motivational factors, were identified through the thematic analysis of students’ answers regarding their choice for entering the nursing degree course. On the empathy scale, females gained higher scores than males. When the age was considered, this difference was shown only for younger students, with young females’ total scores being higher than that of young males. High scores of empathy were positively associated with altruistic motivational factors. A negative correlation was found between external motivational factors and the scores of the Compassionate Care subscale of the Jefferson Scale of Empathy. Conclusions. Knowing the level of nursing students’ empathy and their motivational factors for entering nursing studies is important for educators to implement training paths that enhance students’ relational attitudes and skills and promote positive motivational aspects central for this profession.

Em meio ao crescente envelhecimento natural da população mundial e a falta de distinção entre o processo natural e o patológico do envelhecer surge o ageísmo. O objetivo deste estudo é analisar a expressão do ageísmo nos cuidados em saúde, bem como vem sendo investigado pela literatura e seu desfecho para à saúde do idoso. Os dados foram buscados nas bases: Scielo, Lilacs, PsicINFO, Scopus, Web of Science e Pubmed com a chave de busca: "ageismo AND idoso AND Assistência à saúde" e "ageism AND aged AND healthcare". De 342 artigos, 64 foram selecionados, sendo extraídas as introduções e conclusões dos estudos e criados dois corpus textuais, analisados pela Classificação Hierárquica Descendente (CHD) do software Iramuteq. O primeiro corpus indicou 2 classes, a primeira ligada ao conceito de ageísmo e ocorrência no ambiente de saúde. A segunda, relacionou-se com as formas de investigação desse preconceito. O segundo corpus também resultou em duas, a primeira relacionada às sugestões para novos estudos, a segunda apresentou os aspectos negativos. Diante do exposto, é evidente a presença do ageísmo e suas consequências à saúde dos idosos. Por fim, os estudos apontam estratégias educacionais sobre o ageísmo e envelhecimento como forma de combater esse preconceito.
