Purpose: The objective of this study was to develop a nomogram that can be used to predict the 3-year overall survival (OS) of patients with non-distant metastatic mucinous adenocarcinoma of the colon (NDM-MAC). Methods:From the SEER (Surveillance, Epidemiology, and End Results) database, a total of 1675 patients with NDM-MAC were extracted and utilized as a training cohort. A multivariate COX regression model was developed and visualized as a nomogram in the training cohort. Furthermore, an additional 207 patients were enlisted from the China-Japan Union Hospital, Jilin University, to constitute an external validation cohort. Decision curve analysis (DCA) curves, C-index, AUC values, and internal and external validation calibration curves were used to assess the established model. The training cohort underwent retrospective scoring using a nomogram. The X-Tile software was utilized to determine the two optimal cutoff values, to develop a risk assessment model. Results:Age, T stage, N stage, Tumor site, and the quantity of harvested lymph nodes (Harvested LN) were included as five variables influencing the 3-year OS to construct the nomogram. The calibration curves of the nomogram demonstrate a good consistency between the predicted OS and the actual OS. The C-index for both internal validation and external validation was 0.703 (P < 0.001) and 0.699 (P < 0.001). NDM-MAC patients were divided into three categories according to the risk assessment model. The survival statistics showed a substantial difference (P<0.001) between the subgroups. Conclusion:In conclusion, this study developed a nomogram for NDM-MAC that included 5 variables.

BackgroundMucinous adenocarcinoma (MAC) is a unique subtype of colorectal cancer and its prognostic value remains controversial. This study aimed to compare the clinicopathological characteristics and prognostic differences between patients with MAC and non-mucinous adenocarcinoma (NMAC).Methods674 patients with NMAC, 110 patients with adenocarcinoma with mucinous component (ACWM) and 77 patients with MAC between 2016-2019 were enrolled in the study. Univariate and multivariate Cox regression were performed to analyze the factors associated with prognosis. Predictive nomograms of overall survival (OS) and cancer-specific survival (CSS) for patients with colorectal adenocarcinoma were constructed. Confounding factors were eliminated by propensity score matching (PSM).ResultsCompared with patients with NMAC, patients with MAC were more likely to have a tumor located at the proximal colon, present with a larger tumor diameter, more advanced T stage, higher frequency of metastasis, deficiency of mismatch repair, and elevated preoperative carcinoembryonic antigen. Patients with MAC were related to worse OS (HR=2.53, 95%CI 1.73-3.68, p<0.01) and CSS (HR=3.09, 95%CI 2.10-4.57, p<0.01), which persisted after PSM. Subgroup analysis demonstrated that patients with left-sided or stage III/IV MAC exhibited a comparatively worse OS and CSS than those with NMAC. Furthermore, in patients with stage II with a high-risk factor and stage III MAC, adjuvant chemotherapy was associated with an improved OS, CSS, and RFS.ConclusionCompared with the NMAC phenotype, the MAC phenotype was an independent risk factor for poor prognosis in colorectal adenocarcinoma with worse OS and CSS, particularly patients with left-sided colorectal cancer and stage III/IV. However, patients with MAC can still benefit from adjuvant chemotherapy.

BACKGROUND The colon cancer prognosis is influenced by multiple factors, including clinical, pathological, and non-biological factors. However, only a few studies have focused on computed tomography (CT) imaging features. Therefore, this study aims to predict the prognosis of patients with colon cancer by combining CT imaging features with clinical and pathological characteristics, and establishes a nomogram to provide critical guidance for the individualized treatment. AIM To establish and validate a nomogram to predict the overall survival (OS) of patients with colon cancer. METHODS A retrospective analysis was conducted on the survival data of 249 patients with colon cancer confirmed by surgical pathology between January 2017 and December 2021. The patients were randomly divided into training and testing groups at a 1:1 ratio. Univariate and multivariate logistic regression analyses were performed to identify the independent risk factors associated with OS, and a nomogram model was constructed for the training group. Survival curves were calculated using the Kaplan–Meier method. The concordance index (C-index) and calibration curve were used to evaluate the nomogram model in the training and testing groups. RESULTS Multivariate logistic regression analysis revealed that lymph node metastasis on CT, perineural invasion, and tumor classification were independent prognostic factors. A nomogram incorporating these variables was constructed, and the C-index of the training and testing groups was 0.804 and 0.692, respectively. The calibration curves demonstrated good consistency between the actual values and predicted probabilities of OS. CONCLUSION A nomogram combining CT imaging characteristics and clinicopathological factors exhibited good discrimination and reliability. It can aid clinicians in risk stratification and postoperative monitoring and provide important guidance for the individualized treatment of patients with colon cancer.
