Worldwide, alcohol use and abuse are a leading risk of mortality, causing 5.3% of all deaths. The endocrine stress response system, which is initiated by the peripheral release of corticotropin releasing hormone (CRH) from primarily glutamatergic neurons in the paraventricular nucleus of the hypothalamus (PVN), has been profoundly linked with alcohol use, abuse, and relapse. These PVN CRH releasing (PVNCRH) neurons are essential for peripheral and central stress responses, but little is known about how alcohol affects these neurons. First, we show that intermittent two-bottle choice alcohol consumption blunts endocrine mediated corticosterone response to stress during acute withdrawal in female mice. Conversely, using slice electrophysiology, we demonstrate that acute withdrawal engenders a hyperexcitable phenotype of PVNCRHneurons in females that is accompanied by increased glutamatergic transmission in both male and female mice. Only male mice show a concurrent increase in GABAergic synaptic transmission. We then tested whether chemogenetic inhibition of PVNCRHneurons would restore stress response behaviors in alcohol history female mice in the looming disc test, which mimics an approaching predator threat. Accordingly, inhibition of PVNCRHneurons reduced active escape in hM4Di alcohol history mice only. This study indicates that stress responsive PVNCRHneurons in females are particularly affected by a history of alcohol consumption. Interestingly, women have indicated an increase in heavy alcohol use to cope with stress, perhaps pointing to a potential underlying mechanism in alcohol mediated changes to PVNCRHneurons that alter stress response.

Introduction: Emotional factors are often specified as playing an important role in the context of problematic alcohol use and alcohol addiction. Aims: This study focused on examining the relationship between difficulties in emotion regulation, perceived personal problems, and problematic alcohol use. Methods: 374 participants from the general population in Slovenia and Croatia (34.8% men, 65.2% women) with an average age of 44.28 years (range: 26 to 74 years) completed the Alcohol Use Disorders Identification Test (AUDIT), Individual Problems and Strengths Scale (IPS) and Difficulties in Emotion Regulation Scale – Short Form (DERS-SF). Results: A higher level of problematic alcohol use positively correlated with difficulties in emotion regulation and the assessment of individual problems (p ≤ .010). Multiple linear regression analysis demonstrated that higher AUDIT scores were positively associated with two dimensions of difficulties in emotion regulation: impulse control difficulties (β = 0.22, p = .008) and lack of emotional awareness (β = 0.15, p = .010). Difficulties in emotion regulation completely mediated the effect of individual problems on problematic alcohol use (indirect effect: 0.18, CI [–.06, –.31]; p < .001): higher levels of individual problems contribute to higher levels of problematic alcohol use through the effect of difficulties in emotion regulation. Conclusions: The findings indicate the vulnerability of individuals with difficulties in emotion regulation to problematic alcohol use. Difficulties in emotion regulation are an important factor to consider for understanding the development, maintenance, and treatment of alcoholism and problematic alcohol use.

Excessive alcohol exposure can cause neurobehavioral deficits and structural alterations in the brain. Emerging research evidence suggests that endoplasmic reticulum (ER) stress plays an important role in alcohol-induced neurotoxicity. Mesencephalic astrocyte-derived neurotrophic factor (MANF) is an ER stress inducible protein and is responsible to maintain ER homeostasis. MANF is highly expressed in both the developing and mature brain. We have previously shown that MANF deficiency exacerbated alcohol induced neurodegeneration and ER stress in the developing brain. However, little is known regarding the role of MANF in alcohol induced neuronal damage in the adult brain. In this study, we used a neuron-specific MANF knockout (KO) mouse model to investigate the effect of MANF deficiency on acute binge alcohol exposure-induced neurobehavioral deficits and ER stress. Adult male and female MANF KO mice and littermate controls received daily alcohol gavage (5 g/kg) for 10 days and then subjected to a battery of neurobehavioral tests including rotarods, balance beam, DigiGait, open field, elevated plus maze, Barnes maze, and three-chamber sociability task. Female MANF KO animals were more susceptible to alcohol-induced body weight loss. Alcohol exposure did not affect motor function, however female but not male MANF KO mice exhibited an increased locomotor activity in open field test. Learning and memory was not significantly impaired, but it was altered by MANF deficiency in females while it was affected by alcohol treatment in males. Both alcohol-exposed male and female MANF KO mice displayed increased sociability. Alcohol induced the expression of ER chaperones GRP78 and GRP94 and altered the levels of several unfolded protein response (UPR) and neuroinflammation markers in MANF KO mice in a sex-specific manner. The expression of MANF interacting proteins neuroplastin, PDIA1, and PDIA6 was increased in MANF KO mice, and was further induced by alcohol. In conclusion, alcohol exposure and neuronal MANF deficiency interacted to alter neurobehavioral outcomes, ER homeostasis and neuroinflammation in a sex-specific manner.
